Overview Community Health Partners is seeking a full‑time Advanced Practice Provider (APPs) to join the Advanced Laparoscopic Surgery Associates (ALSA) team. In this role, the APP will provide comprehensive pre‑operative and post‑operative care for bariatric and advanced laparoscopic surgery patients, working closely with attending surgeons and the interdisciplinary care team. Responsibilities include conducting detailed clinical assessments, developing and managing treatment plans, performing medical diagnoses and perscribing appropriate therapies. The APP will support patients throughout the surgical continuum, ensure high standards of safety and quality. Responsibilities The Clinical Nurse (CN) Facility Resource is responsible for providing a high level of clinical support to the acute care areas and numerous procedural areas within Community Health System (CHS). This position delivers patient care through assessment, planning, implementation and evaluation of patient needs. Additionally, will collaborate with interdisciplinary teams, mentor / coach staff members and provide unit based education and support on processes. Accountabilities 1. Rounds on high risk patients that are identified through rounding and early warning scores (including Sepsis Surveillance) within EPIC for all inpatients with a primary focus on patients outside the Emergency Department (ED) and ICUs.2. Assists with various procedures including but not limited to CVAD access and removals (ports), central line and dialysis line insertions and removals, I/O insertions and removals, ultrasound IV starts with CN teaching, level one infusions and procedural sedation.3. Responds to all Code Blues, Code Whites, OB Heme alerts, Code Strokes and Rapid Responses. Validates patient decompensation prior to calling rapid responses.4. Provides nursing care, including medication administration, to patients who are pending transfer to higher level of care.5. Supports emergent/urgent monitored transports to ICUs, SDU, CPCU, MRI, Ultrasound, IR and CT.6. Provides mentorship and coaching to clinical staff.7. Consults with department leadership and team members to support and educate on new or updated processes.8. Performs miscellaneous job related duties as assigned. Make resident rounds at the beginning of each shift and every two hours thereafter to administer quality nursing care. Verify the identification of a resident before administering nursing care or serving food, to assure that appropriate care is being provided to the correct resident. Report missing ID bands to admission coordinator immediately. Make beds, clean bedside and overbed tables. Assure that water pitchers are cleaned, filled with fresh drinking water and within the resident’s reach. Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator.

Position Description Responsible for ensuring the delivery of efficient and effective nursing care while achieving positive clinical outcomes and resident/family satisfaction in accordance with accepted standards of practice, state and federal regulations and licensing requirements. Operates within the scope of practice defined by the state Nurse Practice Act. Responsible for resident care and direction of nursing care during assigned shift; includes staff assignments, mentoring and educating nursing personnel, working with physicians and other medical professionals.
