VITAE - Nurse Practitioner Program The Position As we continue to grow, we are looking for “SNF-ists,” in-house n urse practitioners that will provide care for the geriatric population in a Skilled Nursing and/or Assisted Living setting. What You Will Do As the SNF-ist, you will act as a Skilled Nursing Facility’s leading provider of internal medicine services. You will collaborate with the facility’s interdisciplinary team and physicians, serving as a day-to-day clinical leader while helping oversee the care of all of the facility’s residents. You will work with the entire facility’s patient population - short and long-term residents - overseeing their day-to-day care with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians. As a SNF-ist, you will provide types of care, including but not limited to: Be comfortable and proficient with documentation and CPT coding Evaluation of new-admission visits Coordination of discharge visits Monthly stability visits for long term residents Evaluations for patients exhibiting changes in condition Advanced Care Planning Other Duties & Responsibilities Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease Prescribe all necessary medications and treatments Perform comprehensive physical examinations of patients Develop and implement patient management policies and procedures Facilitate referrals to other healthcare professionals and medical facilities Communicate with attending physicians and/or specialists regarding patient care Be comfortable and proficient with documentation and CPT coding Working with Vitae will provide you with: · A healthy work-life balance with flexibility.
