State Tested Nursing Assistant (STNA) – Skilled Nursing Facility Join our team, where you’ll play a key role in providing high-quality care and support to our residents in a warm and professional environment. Responsibilities: Providing great patient care. Answering patient calls and determining how best to help them. Providing physical support for patients or residents with daily activities and personal hygiene, including bathing, dressing, getting out of bed, using the toilet, walking, standing, or exercising. Turning and repositioning bedridden patients. Ensuring patients or residents receive an appropriate diet by reviewing their dietary restrictions, food allergies, and preferences. Obtaining a wide range of information from physicians, caregivers, and nurses about patient conditions, treatment plans, and suggested activities. Measuring and recording food and liquid intake, urinary and fecal output, and reporting changes to medical or nursing staff. Recording vital signs, including blood pressure, pulse, temperature, and respiration rate, as requested by staff. The unit offers general medical services to all types of patients requiring cardiac monitoring including cardiology, renal, endocrinology, general medicine, pulmonology, gastroenterology, and oncology. Staff on this unit undergo special training and education regarding non-invasive positive pressure ventilation (Bi-PAP, C-PAP, and A-PAP). They also provide infusion therapy, transfusions, injections, and central line care. Patient care assistants on this unit provide support with ADLs, POC glucose testing, and 12 Lead EKGs. 3 South is a dedicated stroke unit and all RNs are NIH stroke scale certified.

The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures.
