The expectation is that the ideal candidates would be able to pick up 3-to-5 PM shifts per week. There will be a weekend differential in pay for those that decide to pick up a weekend shift. Scheduling is truly flexible. The Certified Nursing Assistant is a certified paraprofessional specifically orientated to assist a Private Duty Home Service client with activities of daily living in their place of residence, as specified in the individuals Plan of Care, with adherence to At Home Quality Care’s standards. Duties are limited to those in which the CNA has demonstrated competency. The CNA for Home services is directly supervised by the Private Duty Coordinator as an employee of At Home Quality Care. GENERAL RESPONSIBILITIES: · Observation of client functioning and environment status and reporting changes to the supervisor both orally and in documentation. · Carry out tasks as outlined in each individual clients Plan of Service Care. · Provide assistance with incidental household chores including initiating proper diet, meal preparation, light housekeeping, laundry and linens for client. · Work with clients, family, and agency to create and maintain a safe environment and report any hazards. · Assist client with activities outside the residence, including shopping, errands, and appointments. · Provide companionship, socialization and diversional activities including mental stimulation and emotional support. · Assist with activities of daily living and personal care with the scope of services specified by all deferral and state licensing bodies and according to agency policies, including: bathing, routine hair care, dressing, foot and nail care, routine skin care, shaving, oral hygiene, eliminations, nutrition, feeding, positioning, safe transferring and ambulation, exercise and medication reminding or prompting. · Initiate CPR, Basic First Aid, and contact EMS in case of emergency. · Maintain compliance with agency policy and procedures regarding infection control. The Breast Imaging Nurse Navigator (RN) provides individualized assistance to patients, families, and caregivers to help overcome health care system constraints and facilitate timely access to quality medical and psychosocial care from screening and pre-diagnosis of cancer through placement into appropriate treatment clinics. The Breast Imaging Nurse Navigator works closely with Physicians and staff in all specialty areas to coordinate the patients plan of care and communicates with all members of the team on behalf of the patient. Essential Job Functions: Develope relationships with patients at first point of contact through diagnostic workup, placement into treatment clinics, and/or surveillance programs, as appropriate. Asses patient, family and caregiver needs upon the initial encounter (first abnormal finding or initial diagnostic imaging services) including assessing barriers to care, performing distress screening as appropriate and making referrals to appropriate disciplines and documenting accordingly. Works with the breast cancer-screening progam to identify patients with abnormal screening mammograms who require further workup. Interfaces with outside referring physicians to communicate abnormal screen findings and offer services for further workup. Assist with triaging patients for futher assessment. Track pending pathology for all biopsies performed within Diagnostic Imaging and make referrals based on results. Facilitates the entry of patients into the breast cancer program by reviewing breast imaging results and medical history indicative of breast cancer, triaging and referring newly diagnosed breast cancer cases to service for further care coordination. Coordinate appointments for consults with support services within established service standards, including socialworkers, dieticians, genetic counselors, therapists, and others when needed. Collaborate with DI Operations, access and other teams to obtain medical records, imaging/pathology, and insurance information to expedite initial consultation. Collaborates with clinic staff, research nurses, and physicians to ensure appropriate Breast Imaging for potential and eligible clinical trial patients. Apply knowledge of payer requirements and insurance processes and their impact on referrals for treatment and patient care decisions to set up appropriate treatment, as requested or needed. Responds to voicemail and myChart messages within one business day and answers phone calls in a timely manner. Other duties as assigned, including other leadership roles as required by the center. The RN is expert in assessing and treating and evaluating patient conditions specific to their work area, e.g., anxiety, pain, claustrophobia and underlying disease conditions. Due to the complex nature of pain, RN will evaluate and treat pain in a prompt, caring fashion and reassess after each intervention, charting per UCMC protocol. RN maintains hospital required nursing & ICU unit specific competencies needed to provide proficient nursing care; RN will provide continuity of care for infant, pediatric or adult patients based on area of specialty. This includes assisting in the care of critically ill patients during sedation/procedural encounters. Administering high-risk infusion and transfusion (i.e. chemotherapy medicines).
