Maintain direct interaction with consumers on a regular basis, but in no case, at a frequency of less than once per week. Perform nursing assessments, by direct physical examination, of each consumer upon entry into the program quarterly (at a minimum) thereafter. Develop and implement an individualized plan for nursing service. The nursing care plan must be updated at least annually or whenever there is significant change in the consumer’s condition. Evaluations must become part of the employee’s annual performance evaluation. Maintain documentation of all training curriculums, attendance and post training testing. Must have experience and ability in working with electronic health record management, electronic care coordination and electronic health record interface with external service providers. EDUCATIONAL/TRAINING REQUIRED: Active, unrestricted New York State license as a Registered Professional Nurse. Bachelor’s degree and a minimum of 3 years clinical experience in a hospital or primary care setting preferred. EXPERIENCE REQUIRED/LANGUAGE PREFERENCE: Experience with the Intellectually Disabled population preferred. Managerial experience preferred. Must possess excellent verbal and written communication skills. Ability to work in a team environment. Must be able to work independently and possess sound independent judgment and decision making skills. COMPUTER SKILLS REQUIRED: Proficiency in Microsoft Office Suite applications required.
