RESPONSIBILTIES: Admissions Coordination : Serve as the primary point of contact for new admissions, ensuring all aspects of the process are handled professionally and efficiently. Coordinate pre-admission assessments with the interdisciplinary team to evaluate potential residents' medical and nursing care needs. Verify insurance coverage and eligibility in collaboration with the business office. Clinical Assessment: Conduct thorough assessments of incoming residents, including physical, mental, and emotional health evaluations. Review medical histories, medications, and physician orders to develop initial care plans. Communicate and document any special needs or considerations for each resident. Documentation and Compliance: Complete and ensure accuracy of all admission paperwork, including consent forms, medical records, and regulatory documentation. Maintain compliance with state and federal regulations, including Medicare/Medicaid requirements and HIPAA standards. Update the electronic health record (EHR) system with all relevant information promptly. Resident and Family Support: Welcome residents and their families upon admission, providing a thorough orientation to the facility and its services. Act as a liaison between residents, families, and the healthcare team, addressing questions and concerns with empathy and professionalism. Provide education to residents and families about care plans, medications, and facility policies. Collaboration and Communication: Work closely with the admissions team, nursing staff, and other departments to coordinate the arrival and placement of new residents. Collaborate with external healthcare providers, hospitals, and referral sources to ensure continuity of care. Participate in daily or weekly admissions meetings to discuss incoming residents and prepare for their needs. Quality Assurance: Monitor and evaluate the admissions process, identifying areas for improvement and implementing solutions. Ensure that residents' initial care plans align with their specific needs and that care delivery meets the facility’s standards. Other Duties: Assist in marketing efforts by providing tours and information to prospective residents and their families. Perform other nursing duties as needed, including providing direct care in urgent situations. REQUIREMENTS: Required: Active RN license in New York State, in good standing. Preferred: Bachelor of Science in Nursing (BSN). Minimum of 2 years of experience in a healthcare setting, preferably in a long-term care or skilled nursing facility. Previous admissions or case management experience is highly desirable. Experienced in leadership, supervisory, or teaching position. Ensure nursing staff follow their respective job descriptions. Perform administrative duties such as completing transfers, monthlies, nurse’s notes, reports, etc. as necessary. Admit, transfer, and discharge residents as required. Complete accident/incident reports, as necessary. Participate in developing the agenda for care plan committee meetings. Ensure that rooms are ready for new admissions. Greet newly admitted residents upon admission. Escort to room, as necessary. Do nursing assessments. Make rounds with a physician. Monitors and transcribes physicians' orders, consults and appointments, as necessary. Provide direct nursing care, as necessary. Notify the neighbor’s attending physician and next of kin when there is a change in the resident’s condition. Make independent decisions concerning nursing care. Give/receive the nursing report upon reporting in and ending shift duty hours. Administer professional services such as, but not limited to catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, etc… Participate in the orientation of new neighbors/family members to the facility. Review the neighbor’s chart for specific treatments, medication orders, diets, etc., as necessary. Obtain sputum, urine and other lab tests as ordered. Take and record blood pressure, etc. as necessary. Monitor seriously ill neighbors, as necessary. Administer medications and treatments and document same as necessary. Responsible for knowledge of all safety and fire regulations. Attend Inservice programs, both in and out of facility, to increase professional competence. Responsible for knowledge of all personnel policies. Write, evaluate, and update care plans. All other duties as may be assigned. RESPONSIBILTIES: Employee Health Services: Conduct pre-employment health screenings, including vital signs, TB testing, and immunization reviews. Administer vaccines, including flu shots and other required immunizations, under the direction of the Occupational Health RN or physician. Provide basic first aid for workplace injuries and illnesses and refer employees to appropriate medical care providers as necessary. Monitor employees returning to work after illness or injury, ensuring compliance with clearance protocols. Infection Control and Monitoring: Support infection control efforts by monitoring employee health for symptoms of communicable diseases. Conduct routine screenings for tuberculosis and other infectious conditions as required by facility policies. Educate employees on infection prevention measures, including hand hygiene and PPE usage. Health Record Management: Maintain accurate and confidential employee health records, ensuring compliance with HIPAA regulations. Prepare medication, bring medication to dining room during meal time, and assist residents with medication. If a resident requires medication prior to breakfast, the charge PCA shall go to the resident's room and assist the resident with medication. If a resident requires afternoon or after dinner medication, the PCA shall assist the resident with medication from the med room. The procedure of assistance for injectable medication is as follows: A resident requiring assistance shall come to the med room. The PCA shall assist the resident by handing over the injectable medication to the resident. The resident shall self-administer medication in the presence of the PCA.
