Flex position covers various units as needed based on staffing and operational needs. This position is part time, first shift, is 8-hour shifts (8am-4:30pm), and is a wrap schedule. Week A- Wed/Thurs/Fri, Week B- Mon/Tues. This position has required every 18th weekend and annual holiday components. Scope of Work Plans and prioritizes care for individuals and population of clients, focusing on strategies that will promote optimal health within populations. Provides peer support and role modeling related to the development of life skills and health/wellness to individuals and families during home visits, if applicable. Demonstrates expertise, current knowledge in patient care and management of a caseload of clients of varying complexity and seeks to improve patient, family and health systems/community outcomes through the application of educational concepts/skills and preventive care in a managed care environment. Visit homes to determine client and family needs. Perform home health assessment, if applicable, including blood pressure, pulse, BMI and foot checks. Develops prioritized plan to meet needs, and provides services. Follow-up with families, community agencies and volunteers to evaluate effectiveness of services provided and plan for future needs. Assesses internal and external referrals to identify patient/significant others’ needs, level of intensity, insurance benefits and other patient resources. Develops plan of care and makes recommendations to PCPs, specialists and other members of the health care team regarding care management strategies, identifying strategies to maximize continuity of care across the continuum. Assesses the educational needs of clients, families and members of the health care team and develops and implements appropriate teaching strategies or makes appropriate referrals. Communicates and collaborates with patient/significant others/providers/payers to coordinate services that improve access to appropriate services across the continuum of care and which promotes optimal health in a cost-effective manner. Documents patient data, plan, interventions and outcomes according to department guidelines. Maintains knowledge of current trends and developments in the field by reading appropriate books, journals and other literature, and attending related conferences, seminars, etc. Ensures that processes and services are continuously monitored for quality, cost effectiveness, and efficiency. Engages in process and quality improvement activities. Makes and implements recommendations to improve operational efficiency and to implement new services for areas of responsibility. About the unit We are able to provide care for long term care residents and post-acute rehab patients. Nurse-patient ratios vary depending on the shift and the population you will be serving. Each designated unit focuses on patient specialties, such as general rehab, neuro rehab, traumatic brain injuries and cancer. About the Unit The PACU at Helen DeVos Children's Hospital has a total of 12 bays. The Surgical Prep Recovery Unit (SPRU), which is our preop and phase II space, has 25 beds. Staff is cross trained in each area and will be expected to flex in and out of each space depending on need. We have both ICU level cases and outpatient cases. This unit specializes in the care of those with oncology diagnoses and a surgical population of head and neck cancers. About the Position Mixed Weekend/Weekday Infusion RN Role in the infusion clinic at Lemmen Holton Cancer Pavilion. Serving the oncology and general populations by providing a wide range of infusion services and medication as well as blood product transfusion. Two-week rotating core schedule Week One: Wed 0830-2100, Fri 0830-2100 Week Two: Tues 0830-2100, Wed 0830-2100, Fri 0700-1930 3 MHC is a 24 bed Prep/Recovery unit at Meijer Heart Center that prepares and recovers both outpatients and inpatients for their Cath and Electrophysiology Lab procedures. The unit is open from 6am – 9pm. The majority of 3 Heart outpatients are discharged the same day and are cared for by the same staff from arrival to discharge. We perform a wide range of procedures on 3MHC, including, electrophysiology, diagnostic and interventional cardiology, structural heart, and peripheral. Team members hired onto 3MHC will gain knowledge in all of these specialties. In addition, we perform diagnostic and follow up care to our heart and lung transplant patients, creating an opportunity for long term relationships with this patient population. Our team are experts at IV starts, time management, and managing vascular access sites. Nurses are often drawn to this unit due to the camaraderie and true sense of teamwork. We have a small team, and work alongside nurses, nurse techs, doctors, PA's, and cardiovascular technicians. Our clinic includes Burn, Orthopedic Trauma, Acute Care Surgery and Physical Medicine and Rehabilitation. Our goal is to provide convenient and comprehensive care to patients who have suffered a traumatic injury. For convenience of the patient, we have the ability to offer multiple appointments with different trauma specialty providers in one location.
