Job Description Responsible for working with other healthcare professionals to help treat patients with various injuries, illnesses or disabilities. Monitors patients, administers medications, keeps records, consults with healthcare providers, educates patients and more. Principal Accountabilities Maintains accurate, complete health care records and reports. Administers medications to patients and monitors them for side effects and reactions. Prescribes assistive medical devices and related treatments. Monitors, reports, and records symptoms or changes in patient conditions. Assesses, implements, plans, or evaluates patient nursing care plans by working with healthcare team members. Records patient vital signs and medical information. Performs other related duties as assigned. Required QUALIFICATIONS Graduate of an accredited Nursing Program. Current state licensure as a Registered Nurse. BLS certification. Preferred Bachelor’s degree in nursing. All ASN must complete their BSN studies within five years of their start date. Master’s degree in nursing or related field. 1+ year of experience in Nursing or relevant area. ACLS certification. Serenity Hospice is looking to add a dedicated and compassionate RN Case Manager to delivering exceptional care, comfort and support to patients. They will develop plans for individual needs, perform regular check-ups and communicate with family members as needed. This position will oversee Hospitals throughout Morris County . PURPOSE AND SCOPE: The professional registered nurse Home Therapies RN CAP 2 may be an entry level designation into the Clinical Advancement Program (CAP) for new employees who meet the RN CAP 2 criteria or attained through advancement from RN CAP 1. This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. This may include delegation of appropriate tasks to direct patient care staff including but not limited to RNs, LVN/LPNs and Patient Care Technicians. As a member of the End Stage Kidney Disease (ESKD) health care team, this position participates in decision-making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and facility operations. PRINCIPAL DUTIES AND RESPONSIBILITIES: All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations. Performs all essential functions under the direction of the Supervisor and with guidance from the Educator, Preceptor or in collaboration with another Registered Nurse. Performs ongoing, systematic collection and analysis of dialysis data for assigned patients and documents in the patient medical record, makes adjustments or modifications to treatment plan as indicated and notifies Supervisor or physician as needed. Assesses, collaborates and documents patient/family’s basic learning needs to provide initial and ongoing education to patients and family. Directs and provides, in collaboration with the patient, home care partner, direct and ancillary patient care staff, all aspects of the provision of safe and effective delivery of dialysis therapies to assigned patients. Administers medications as prescribed or in accordance with approved algorithm(s), and documents appropriate medical justification and effectiveness. Initiates or assists with emergency response measures. Serves as a resource, leader, coach, mentor and role model for new and incumbent employees by setting examples of appropriate behavior, work habits and attitudes towards patients, co-workers, supervisors and the company at the facility and area level. Ensures correct laboratory collection, processing and shipping procedures are performed and reschedules missed or insufficient laboratory collections. Identifies expected outcomes, documents and updates the nursing assessment and plan of care for assigned patients through collaboration with the Interdisciplinary Team. Ensures patient awareness related to transplant and treatment modality options. Assists in the identification, evaluation, selection and education of Home Dialysis candidates and Home Partners. Performs assessment and identifies barriers of the Home Dialysis candidate’s home environment and partner/ family readiness and ability to perform dialysis treatments in the home. Trains home dialysis patients and / or Home Partners on the safe, effective operation and maintenance of all Home Dialysis equipment and treatment supplies through a formal standardized Home Dialysis Training Program. Participates in education and quality improvement projects at the facility and area level as directed by Supervisor. May serve as a Preceptor to new employees. Required to complete CAP requirements to either maintain or advance. Performs all other duties as assigned by Supervisor. PHYSICAL DEMANDS AND WORKING CONDITIONS: The physical demands and work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. The Nursing Supervisor performs responsible supervisory and professional work in directing and coordinating all nursing units. The individual must have had advanced training and broad nursing experience including supervisory duties. The work requires administrative ability, knowledge of training techniques, ability to adapt techniques and procedures in accordance with unusual situations, and the exercise of initiative and good judgment. The work is carried out according to standards of professional nursing practices, facility policies, rules and regulations, and State and Federal regulations. RESPONSIBILITIES/ ACCOUNTABILITIES: 1 Verifies daily staffing and redistribution, as necessary; 2 Collaborates with all charge nurses daily, to exchange information regarding resident care, resident condition and all matters affecting resident well-being (i.e. admissions, discharges, transfers, out of or within the building), makes recommendations regarding nursing process, and communicates concerns to supervisors of other departments; 3 Makes rounds, morning and night, as time allows, to assess residents condition, giving special attention to critically ill residents; 4 Ensures that the individual nursing care plan is followed for each resident; 5 Performs medication passes and/or direct resident care as needed; 6 Participates in resident care committee meetings; 7 Reinforces the training and orientation of nursing personnel and provides positive reinforcement; 8 Answers any/all emergencies throughout the shift (i.e., change of resident condition or pronouncement of death); 9 Assists in assuring a safe and sanitary environment for residents, employees, and visitors; 10 Assures that the building is secure during the evening and night hours by checking doors and alarms according to nursing facility protocols; 11 Reviews incident and investigative reports for accuracy and completeness; 12 Monitors meal service on the units and in the dining areas to identify issues requiring intervention and to assure resident meal satisfaction and nutritional adequacy; 13 Assists in development and implementation of nursing facility Quality Improvement Program; 14 Ensures that residents and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals’ needs and rights; 15 Performs other duties as requested. SPECIFIC EDUCATIONAL/ VOCATIONAL REQUIREMENTS: 1 The Nursing Supervisor must be a graduate of an accredited school of nursing and there must be current registration with the professional board of nurse examiners of the state. 2 Experience in the nursing field. 3 Supervisory experience in long term care is required.
