Newport News, Virginia Overview Works under the supervision of Registered Nurse/LPN and Nurse Manager/Director in personal care of hospital patients and their environment (including those needing special precautions) and performs specified treatments. In addition, this position must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served in assigned unit. The individual must demonstrate knowledge of the principles of growth and development over the life span and provide the care needed as described according to the departmental policy and procedure. Promotes and maintains safe work environment for staff as well as our customers, incorporating national patient safety initiatives. What you will do Demonstrates understanding and knowledge of the technology systems necessary for patient data collection. Demonstrates accountability for ensuring availability of resources (equipment, instruments, supplies) necessary to ensure positive patient outcomes. Maintains clean, safe and orderly work/patient environment by cleaning work stations/areas, equipment; stocking supplies; placing equipment in proper storage areas. Demonstrates the ability to follow plan of care to include performing treatments and procedures competently/independently under direction of RN/LPN. This may include the following procedures dependent upon unit/facility competency requirements such as enemas, vital signs, ice application, compresses, perineal care, rectal tube, sitz bath, unsterile sites, strain urine for stones, specimen collection, etc. Demonstrates the ability to follow the plan of care that assures patients receive consistent, competent, and ethical care, within legal standards of practice, while maintaining privacy and support for the patient’s value system. Provides care consistent with both verbal and/or written instructions. Demonstrates competent understanding of assigned plans of care within appropriate scope of practice and responsibilities as directed by the RN/LPN to include accurately completing all required documentation. Participates positively in training and support of all team members, volunteers and students as needed. The RN may work in a variety of settings and roles within Riverside Health System. The RN utilizes communication, critical thinking, problem solving, and decision making to create a care plan for each patient based on physiological, psychosocial and cultural diversity. Responsible for supervising and delegating non-professional personnel working directly with him/her to ensure optimum patient care. Must demonstrate competence in dealing with age related needs of the specific patient populations. Works under the direction of leadership and a multi-disciplinary team to provide a comprehensive range of services or clinical care associated with and in accordance with standards of excellence established by Riverside Health system and facility goals and strategic plans. What you will do Demonstrates ability to assess patient clinical ands psychosocial needs by applying standards of practice as it relates to the specialty or patient population served in the Critical Care/Telemetry units/departments (see unit specific competencies and equipment checklists). Partners with patients and families to develop, implement and evaluate plan of care, treatments and procedures that contribute to the physiological and psychosocial outcomes of the patient. Assesses and incorporates evidence-based standards of practice. Collaborates with interdisciplinary team in the coordination care. Facilitates safe patient transitions to the next care setting as part of plan of care. Demonstrates a sense of urgency to facilitate patient throughput to assure patients receive consistent, competent, and ethical care. Focuses on quality outcomes of patient care when addressing customer’s needs to include current evidence based practice and quality measures. Demonstrates understanding of diverse patient care populations and ability to modify education method to ensure patient/family comprehension. Educates by assuming responsibility to individualize and coordinate patient education, and discharge specific education that includes family/caregiver. Evaluates for patient comprehension and documents. Accurately completes all required clinical and quality documentation consistently per RHS standards and regulatory requirements. Ability to delegate and prioritize tasks appropriately to facilitate quality patient care and ensure patient care needs are met. Able to recognize and respond to emergency situations.
