A licensed professional nurse under the supervision of a Registered Nurse who provides nursing care and services to residents in a long-term care setting. Qualifications: • Graduate of an accredited school of nursing or an approved LVN program • Current nursing licensure in good standing with the State licensing board • Strong communication and organizational skills • Ability to provide quality patient care in accordance with applicable standards, policies, and procedures • Ability to identify and report problems, use initiative and good judgment to reach quality decisions • Comprehensive knowledge of applicable nursing principles and practices to ensure the quality of care standards for residents are • Possess the willingness to work with professional and non-professional personnel. Proficiency of the English language including the ability to speak, read, write, and comprehend the language with minimal to no difficulty • Valid CPR certification General Duties and Responsibilities: GENERAL • Provides nursing care as prescribed by physician/health care professional in accordance with the legal scope of practice, any Board of Licensing restrictions, and within established standards of care, policies, and procedures • Administers professional services and provides care consistent with allowing residents to attain or maintain his or her highest practicable physical, mental, and emotional well-being. Provides clinical data and observations to contribute to the nursing plan of care CLINICAL • Admits, transfers, and discharges residents in accordance with policy and procedure • Orients new residents/families as necessary • Gives/receives nursing reports upon beginning and ending of assigned shift. Reviews daily patient report and assignments with Nursing Assistants upon the beginning of assigned shift • Assures that the unit area is maintained in a clean and safe manner for resident comfort and convenience. • Conducts daily resident rounds to observe the resident’s physical and emotional status • Collects clinical data and reports significant clinical findings according to policy • Performs manual and technical skills according to the scope of practice under the direction of a Registered Nurse • Records care information accurately, timely and concisely. Completes all required documentation including resident observations, interventions, and patient response(s) in the medical record in accordance with policy • Assists in developing, reviewing, revising, and updating resident Plans of Care as indicated. Contributes to the evaluation of the patient’s progress towards specific goals and the adjustment of the nursing plan of care as necessary • Receives and transcribes orders accurately from attending/alternate physician. • Assists in making rounds with the attending physician, as necessary. • Prepares/administers medications as ordered by the physician and within the legal scope of practice. • Completes medical treatments as indicated and ordered by the physician. • Evaluates response and report adverse reactions to medication or treatments in accordance with the policy. • Assures that narcotic and controlled drugs are secure, inventory counts are accurate and narcotic register documentation is timely and accurate • Orders prescribed medications, supplies and equipment as established by the facility’s policies and procedures. • Requisitions and arranges for diagnostic and therapeutic services as ordered by the physician. • Notifies the attending/alternate physician, as well as the resident’s legal guardian/representative, when the resident becomes critically ill, injured, etc., or has a significant change in condition as outlined within established policies. • Notifies the attending/alternate physician of resident incidents/unusual occurrences. Completes Accident/Incident Report forms as indicated and documents relevant information in the resident’s medical record as outlined in established policies and procedures. Initiate investigation of accidents and unusual occurrences • Informs RN Supervisor or RN in charge of the facility of significant changes in resident condition, unusual occurrences, incidents, or accidents as indicated and per facility policy and procedure • Attend various facility meetings (care conferences, stand-up meetings, staff meetings, Medicare meetings, discharge plan meetings, etc.) as required. • Reports allegations of abuse or unusual occurrence per State and Federal Regulations and facility policy. Assists with a gathering of information relative to the event. • Initiates emergency procedures (including basic CPR) and provide emergency patient treatments • Participates in the facility’s infection control program for monitoring communicable and/or infectious disease among residents and staff. • Participates in procedures for reporting hazardous conditions or equipment. • Assists in orientation of newly assigned personnel as requested. • Assumes Charge responsibility following orientation period as requested. • Performs other position-related duties as assigned. • Supervise CNAs SUPERVISION • Make resident rounds to ensure appropriate care is being rendered, identifying, and making corrections as necessary • Meet with nursing personnel to assist in identifying and correcting problem areas and/or the improvement of resident care • Assist the Charge Nurse in revision and distribution of nursing assignments as needed • Assist in the supervision and direction of nursing personnel • Participate in disciplinary action and evaluation of nursing personnel CONSUMER SERVICE • Presents a professional image to consumers through dress, behavior, and speech. • Treats residents/family members with dignity and respect • Responds to Resident and family concern(s) and assists in resolving grievances. Records grievances per facility policy ADDITIONAL DUTIES • Participate in the facility's Quality Assurance (QA) program as requested or needed. Assist in correcting findings and deficiencies from the Department of Health Services, Consultants, and the Quality Assurance Team as directed by the Director of Nursing. Actively participates in Quality Assurance Performance Improvement teams and activities • Ensures the protection of health information as required by the Health Insurance Portability and Accountability Act (HIPAA) • Participates in the facility’s Infection Control Program for monitoring communicable and/or infectious disease among residents and staff • Participate in procedures for reporting hazardous conditions or equipment. • Orient new staff members upon request • Participate in the facility's quality assurance (QA) program as requested or needed. The Clinic Nurse Manager holds 24-hour accountability for the quality of patient care, efficiency of operations, and human resources management for area(s) assigned. The Manager is responsible for the provision of safe patient care, in a manner that is consistent with the professional standards and hospital policies. The Nurse Manager holds accountability for the quality of patient care, efficiency of outpatient operations, staffing, and human resources management for the assigned outpatient departments. Patient care and services are carried out in collaboration with hospital administration and the medical staff and in compliance with organizational goals and appropriate regulatory agency requirements. Oversight includes the management of providing therapeutic care to patients and their families in the Neurosciences outpatient clinics, including but not limited to triage, procedures, infusion, preoperative clearance, and postoperative care. Serves as the primary competency validator/educator for the clinical staff. In addition, as an integral part of the multidisciplinary team, the Clinic Nurse Manager utilizes the nursing process to provide therapeutic care to patients and their families in the outpatient setting, when required. Ensures patient's right to privacy, safety, and confidentiality is maintained. Maintains a safe environment in accordance with standards, policies, and safety regulations. Ensures compliance with infection control policies. Complex, independent problem solving when overseeing a team of clinical staff and patient care coordinators. Other duties as assigned. Contributes to the development and implementation of a unit-based performance improvement program which complies with Joint Commission, Title 22, and other applicable regulatory agencies. Participates in establishing systems within the departments which support and practice shared governance management philosophy. Contributes to the development of customer service standards and expectations. Our team of professionals helps improve quality of life by addressing the physical, emotional, and spiritual needs of our patients. Through this interdisciplinary approach, we seek to comfort the heart, mind, and body. The Hospice Registered Nurse is an experienced registered professional nurse who initiates and coordinates the hospice care plan plans, organizes, and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. Essential Job Responsibilities: The RN provides care to patients who have been diagnosed with a terminal illness. The Hospice RN regularly reviews and updates the plan of care. Performs prescribed medical treatments, including pain management and symptom control, conducts assessments and evaluations, provides education and supportive care to patient and family. Completes an initial, comprehensive, and ongoing assessments of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). Provides professional nursing care by utilizing all elements of nursing process. Assesses and evaluates patient’s status by: Writing and initiating plan of care, regularly re-evaluating patient and family/caregiver needs, participating in revising the plan of care as necessary. Uses health assessment data to determine nursing diagnosis. Develops a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions. Includes the patient and the family in the planning process. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. Counsels the patient and family in meeting nursing and related needs. Provides health care instructions to the patient as appropriate per assessment and plan. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. Acts as Case Manager when assigned by Clinical Supervisor/Nursing Supervisor and assumes responsibility to coordinate patient care for assigned caseload. Completes, maintains, and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. Communicates with community health related persons to coordinate the care plan. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. Provides and maintains a safe environment for the patient. Assists the patient and family/caregiver and other team members in providing continuity of care. Works in cooperation with the family/caregiver and hospice Interdisciplinary Group Members to meet the emotional needs of the patient and family/caregiver. Attends interdisciplinary group meetings. Position Qualifications Registered Nurse with valid CA state license. Hospice experience preferred. At least one year of recent hospice experience along with a strong foundation of acute care experience. Maintains a current CPR certification. Valid driver’s license, auto insurance, reliable transportation, and willingness to drive to patient locations.
