Develops and prioritizes an outcome directed plan of care to assigned patients and changes as necessary. Performs clinical and technical aspects of care in accordance with established policies, procedures, standards of practice and within scope of the State Nurse Practice. Evaluates the effectiveness of nursing interventions. Modified the plan of care focusing on age and development status and patient outcomes. Reviews the plan of care and consults with the physician and other health care providers to adjust the plan to assure optimum patient outcomes within the scheduled timeframe. Local travel only. Mobile Hospital at Home Registered Nurse provides inpatient care to acutely ill patients within their residences as an extension of in-hospital care. Responsible for providing and coordinating comprehensive patient care through the nursing process to deliver safe, therapeutic care in accordance with established standards, policies, and procedures. Using evidence-based practice, clinical decision making, compassion, and skilled communication leads efforts to create the safest patient environment and the best patient experience across the continuum. The Mobile Hospital at Home Registered Nurse provides hospital-level acute care in a patient’s home within a defined radius around designated hospitals as part of the Advocate Hospital at Home team, which may include the following: Physicians, Advanced Practice Providers, Pharmacists, Case Managers, Social Workers, Physical Therapists, and other operational staff according to the Hospital at Home Care Model. We’re Looking For: Completion of an accredited or approved program in nursing Certification / Registration / License Registered Nurse license issued by the state in which the team member practices Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA). Required to have a valid driver’s license issued by the Division of Motor Vehicles and acceptable vehicle operator record Travel required within designated geographic area Work Experience Minimum of two-years of recent nursing experience in an acute bedside setting or emergency nursing Knowledge / Skills / Abilities Demonstrates progression to provide holistic patient care in the home and the ability to view clinical situations from a patient and family focus. Demonstrates progression to recognize situational changes while providing care in the home that requires unplanned or unanticipated interventions, and responds to situations with confidence, timeliness and flexibility.
