This unit provides complex, high-acuity care with a strong cardiac focus including post–cardiac intervention monitoring, arrhythmia management, and titration of cardiac-related therapies. The care may include, but is not limited to, the following: obtaining and recording in the medical record: blood pressure, pulse, temperature and pulse oximetry, drawing blood, EKG, administration of oral medications, administration of maintenance fluids, administration of tube feedings, transporting patients, obtain urine and stool specimens, perform point of care testing, assist with procedures consistent with the scope of the employee’s position (i.e.. dressing changes, urinary catherization, bathing, dressing, grooming, toileting, ambulating, transferring, feeding, observing, and reporting the general well-being of the person(s) to whom they are providing care). Responsibilities: The LPN must read and interpret documents such as doctors’ orders, medication names, and lab tests. Perform focal assessments for established inpatients. Provide general basic nursing care, such as positioning patients, lifting and turning, assisting in use of bedpan, commode, urinal or bathroom, and weighing. Provide exceptional customer service that aligns with the Planetree philosophy. Perform routine, uncomplicated treatment procedures including enemas, application of hot and cold compresses, dressing changes, wound cleansing, obtaining vital signs including orthostatic vital signs and rectal temperatures and splinting of extremities. Patients on 6A enjoy private rooms, is very well equipped, clean and organized. 6A is a 26 bed unit where the acuity ranges from Med Surg to Intermediate Care and receives a variety of patients including Neuro and Neuro like conditions. They have great teamwork, participates in the Nurse Residency Program and Shared Governance. The Registered Nurse provides professional nursing care to patients within a assigned unit with a commitment to quality and safety. Professional nursing practices, requiring specialized knowledge judgment and skill are performed using the process in accordance with hospital policy, procedures and nursing philosophy. Responsibilities: Provide direct, professional nursing care to specified population of patients in the acute care setting. Develops a patient centered plan of care based on individualized needs. Maintains confidentiality of the patient medical record. Counsel and educate patients and their care partner on treatment plans. Administer medication following the five rights of medication administration. Provide wound care and dressing changes, as applicable. Read and interpret patient information, making treatment decisions, when appropriate. Consult with physicians to decide on best treatment decisions when appropriate Performs nursing interventions to achieve optimal patient outcomes. Observe and record patient behavior and response to nursing interventions. Demonstrates quality, safe and effective patient care.
