RN UNIT MANAGER Polaris Healthcare and Rehabilitation Center, a beautiful 100 bed facility with a small vent unit, located at the Milford Wellness Village, has an unique opportunity for the right person. This full time position is scheduled Monday through Friday! We are looking for a RN Unit Manager who has strong leadership qualities and excellent communication skills to manage the unit. Responsibilities for the RN Unit Manager Include: Utilizes the nursing process to assess, plan, implement and evaluate resident care. Assess signs and symptoms indicating changes in the patient’s condition. Documents the patient’s plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions. Performs interventions according to identified priorities, plan of care, and the hospital policies and patient care outcome standard. Revises the plan of care according to evaluation, changes in medical plan of care, and effective/ineffective nursing interventions. Uses clinical judgment in evaluation activities to meet patient care needs of an assigned unit/floor including establishing priorities. Qualifications for the RN Unit Manger: Must be a licensed RN with valid license. Familiarity with Federal, State, and local long term care requirements. Two years professional nursing experience.
