Day (United States of America) Registered Nurse- Hospice (Field Case Manager) The Field RN will be responsible for coordination and implementation of the overall plan of care for the assigned patients in various hospice settings to assure the best possible quality of life is attained. Overview The Licensed Practical Nurse will focus on providing high level patient-centered care in a physician office setting by measuring and recording vital signs, placing patients in exam rooms, preparing patients for exams/procedures, assisting the health care provider with exams/procedures, providing patient support during exams, maintaining medical supply inventory, and stocking exam rooms. May be required to float to other UFHP practices, as needed, for staffing. Qualifications Minimum Education and Experience Requirements: Licensed LPN eligible to practice in the State of Florida.
