FLSA Classification: Exempt Department: Clinical Reports To: Clinical Manager Direct Reports: N/A POSITION SUMMARY: The Case Manager Registered Nurse (RN) is responsible for partnering with the Clinical Director and team to administer and coordinate skilled nursing care to patients requiring home care services. Services are provided in accordance with physician’s orders, under the direction and supervision of the Clinical Director and in compliance with applicable laws and regulations and the policies of the organization. RESPONSIBILITIES: Clinical Care: Provides direct care and case management of assigned patients, including initial assessment and development of plan of care, as well as oversight and administration of prescribed medications and treatments. Maintains and updates schedules of all disciplines in patient’s home. Consistently assesses and evaluates patient’s status and goals and revises the plan of care as necessary. Communicates adjustments to plan of care to all team members in the appropriate time frame to support consistent delivery of care. Complete authorization modifications in a timely manner, where needed. Initiates and applies appropriate preventative, therapeutic, and rehabilitative procedures and maintains communication regarding patient care with the physician, supervisory personnel, and other professional staff involved in patient care. Consistently monitors and manages patient medications; maintains an accurate medication profile; coordinates prescription refills with physicians and pharmacies and picks up medication refills as needed. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet, and other instructions as ordered by the physician. Recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Provides and maintains a safe environment for the patient. Reports unsafe conditions, complaints and incidents on behalf of patient as appropriate. Observes and supervises Home Health Aide & LPN to evaluate performance and quality of care. Accepts responsibility of assignment to perform a specialized procedure, such as IV therapy, when qualified with appropriate training and proven competency in the delivery of the therapy. Uses supplies and equipment effectively and efficiently. Maintains equipment per agency policy. Fulfills the obligation of assigned patient case load including tracking of patient status when transferred, following up with other healthcare facilities providing care to patients, resuming care of patients when appropriate. Adheres to HIPAA laws and maintains patient confidentiality always. Other duties assigned by the case manager/clinical management, i.e., performing field supervisions of care team members as required by agency policies. Performs other duties as assigned. Also directs and guides patient teaching, activities of ancillary personnel while maintaining standards of professional nursing. Applies knowledge of illnesses, injuries and diseases and uses. Provides written assessment of patient on admission documents nursing interventions, patient response, effectiveness and complications and summarizes information on discharge. Summarizes assessment data and makes a nursing care plan which is written and communicated to other patient care providers. Implements clinical and technical aspects of care and nursing orders in accordance with established policies and procedures. Intervenes with proper technique, procedure and safety precautions to meet individual needs of patient. Functions within limits of State Nurse Practice Act, hospital and nursing department policies and procedures. Assesses, plans and implements instruction for patient and family need for patient and family with an emphasis on dialysis treatment. Reports patient care or management problems to Clinical Coordinator and/or Supervisor when necessary and investigates and intervenes in problem situations reporting appropriate information. Makes outpatient assignment based on acuity and staff qualifications and experience. Coordinates patient scheduling, unit routines, providing guidance to staff, orienting new staff, assuring presence and working order of emergency equipment. Understands and applies theoretical applications of hemodialysis and routines of treatment procedures. Recognizes and implements corrective procedures for complications of treatment. Performs on-site specific lab testing maintaining appropriate records and controls. Demonstrates understanding and application of dietary and medication management issues for ESRD patients. Demonstrates a working knowledge and safe use of medications specific to the Chronic Care patient population. Identifies own learning needs and seeks opportunities for self-growth, using and sharing skills gained through continuing education classes. Completes annual nursing competencies. Evaluates for age-appropriate behavior motor skills and physiological norms of patients, recognizing a patient’s psychological development and modifies approaches for patients. Interview/communication techniques are age-appropriate. Qualifications Education Associate's Degree Nursing required Bachelor's Degree Nursing preferred Licenses and Credentials Registered Nurse - State License BLS - required Experience 2-3 years Hemodialysis required Knowledge, Skills and Abilities - Must be able to demonstrate adequate technical skills and judgment to function unsupervised during on-call, second shift, and weekend hours. - Ability to tactfully communicate with both internal and external customers. - Ability to handle stress and work in emergency situations. - Ability to use of fingers and hands to operate all department-related equipment as described on the Department checklist. - Additionally, must use beeper, telephone, calculator, fax, and other related office equipment. - Ability to Stand and walk approximately ninety percent (90%) of shift, with frequent climbing, bending, reaching, stooping, squatting, and helping to lift objects and/or patients ranging from five (5) to three hundred (300) or more pounds. - Ability to bend, twist, crouch, squat, pull, push, reach, and remain stationary for short period of times and perform repetitive motions. - Ability to use hands, fingers, arms and wrists, hearing, smell, and verbal skills are required in order to perform job duties. - Ability to differentiate colors.
