Assesses assigned case load of home health care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, Interdisciplinary Team reports, after hours reports, and on-site evaluations. Assumes primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment. Assesses patient needs and obtains data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status and incorporate that data into the plan of care. Makes the initial evaluation and re-evaluates the patient’s nursing needs during each visit. Makes follow up visits according to patient, family, and facility needs. Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care. Revises the plan of care in response to identified patient care issues and notifies the Clinical Supervisor, physician, facility, and other team members. What is Required? Current, unrestricted state RN licensee 1+ year experience in a clinical care setting as a nurse Able and willing to travel within branch/office coverage area. Job Description: Where You’ll Work In this hybrid role, you’ll support patients throughout Middlesex and Essex Counties, MA , spending about 70% of your time in the community meeting patients where they are , and 30% working remotely . The RNCM manages a panel of rising and high intensity members to support integrated chronic disease and behavioral health care for the members. The RNCM collaborates with members to create a care plan and oversees progress to the plan, frequently reassessing needs, coordinating with providers and specialized resources, and partnering closely with the Community Health Partner to build trust and demonstrate advocacy. Responsibilities: Receive members from the engagement and care team, clearly communicating program expectations, including duration and goals. Complete self-efficacy and condition-specific screeners during the assessment and intake phase, along with behavioral health screeners like PHQ-9, GAD-7, AUDIT, and DAST-10 to identify behavioral health needs. Conduct in-person clinical examinations when appropriate and collaborate with care team members to determine member placement in programs of varying intensity. Prepare for and actively participate in case conferences, leading discussions when necessary. Develop a care plan in collaboration with the member and address social needs with the support of the Community Health Partner. Conduct regular clinical visits and follow-ups per program guidelines, monitoring routine therapeutic interventions and addressing member needs promptly. Collaborate with the care team to support a panel of assigned members, providing clinical assistance in health maintenance, chronic disease management, and co-occurring psychiatric disorder support. Perform medication reconciliation, administration, compliance, and education as part of member care. Address quality gaps prioritized by the contracted company and ensure thorough chart documentation and coding (ICD or CPT) to validate gap closures. Utilize care facilitation tools, electronic health records, and scheduling platforms to gather data, document member interactions, organize information, track tasks, and communicate with team members and community resources. Support members in achieving their care plan goals through coordinated and comprehensive care efforts. Essential Functions: Mentoring/Training: (approximately 20 hrs.) 1. Supervise, train and evaluate new Advanced Practice Providers (NP and/or PA). 2. Perform chart reviews based on an established schedule. a. All charts for the first month of a new hire’s employment. b. Up to 10 chart reviews for each month following. 3. Coordinate MAVEN Services which include: a. Coordinate CME educational sessions with site medical directors b. Meet with APP assigned MAVEN mentor at designated times to begin after new APPs 3rd month of employment 4. Observe and shadow APP 5. Evaluate APP based on accepted competencies which will include: a. History taking b. Physical exam skills c. Analyzing data d. Management/ plan formation e. Procedures f. Problem solving hazards and biases g. Medical practice and principles 6. Provide constructive feedback to APP for the course of two years and possibly longer as the APP develops into the role. 7. Support organizational compliance and training requirements and work with APPs to complete any required or newly required trainings. 8. Job Description: I. Position Summary: The RN / Licensed Practical Nurse (LPN), in collaboration with a Registered Nurse, utilizes the nursing process of assessment, planning, implementation, and evaluation to ensure quality nursing care and to maximize the quality of life for the patients. The LPN is an integral member of the clinical support team and collaborates with other multidisciplinary members to meet the clinical needs of the patients. Participates in performance improvement projects Participate and assists with unit activities as needed (i.e. assist with serving meals). Serves as a coach and mentor to patient care associates. May be assigned charge nurse responsibilities with RN oversight. Responsibilities may include: Giving the shift report to the incoming staff Provide clinical/supervisory oversight to the unit during a shift Delegate assignments to the team assigned to the shift Audit/assure completion of admission or discharge during shift with the RN’s cos-signature Attend rounds (wound, physician, etc) If acting in the capacity of charge nurse, may develop patient care assignments (appointing other staff, RN, LPN or PCA to implement an established care plan for a designated period of work time), create schedules, and contribute to performance evaluations and educate other staff on HSL policy and procedure. Qualifications A graduate of an approved school of nursing is required Current Massachusetts license as an RN/ LPN in good standing Previous experience preferred Understand the philosophy and support the model of care at HRC required Must be a professional, proactive, collaborative, conscientious, and results-oriented individual.
