A clinically competent, registered, professional nurse who prescribes, delegates and coordinates nursing care to the acutely ill patient through the nursing process of assessment, planning, intervention, implementation and evaluation. Also responsible for phone triage, abnormal test result delivery and corresponding care instructions. Location: Woodridge/Napervile Responsibilities: Performs all aspects of patient care by utilizing evidenced based practice in a manner which optimizes patient safety and reduces the likelihood of medical/health care errors. Follows the six medication rights and reduces the potential for medication errors, administering medications based on the age and clinical condition of the patient. States rationale for clinical decisions based on critical thinking. Assesses, analyzes, validates and integrates data to assess patients' physical psychosocial, rehab, safety, learning, spiritual, cultural, age-specific, special risk and self-care needs. Identifies patient care needs, problems and variances. Identifies patient care needs, problems and variances and incorporates into care plan. Identifies the patient's learning needs, abilities, preferences and readiness; also, identifies support systems, coping mechanisms and barriers to learning. Explains procedures, therapies and treatments to patient and family, evaluates and documents patient and family response to teaching. Uses appropriate resources to provide education. Involves patient/family in the formation of the plan of care. Develops, implements and updates an individualized plan of care based on identified patient care needs. Includes nursing, teaching, continuum of care and discharge planning. Adequately assesses and reassesses pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management. Identifies and uses the results of quality improvement activities to initiate changes in nursing practice and in the healthcare delivery system. Participates in shared governance, committee work, and practice/department projects. PURPOSE AND SCOPE: The registered professional nurse Home Therapies RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). Essential Job Functions: • Depending on location, performs phlebotomy and specimen processing as directed by delegating provider. • Administers, including but not limited to, intramuscular injections, subcutaneous injections, intradermal skin testing, topical medications, ophthalmic drops and nasal spray. • Depending on location, performs ECGs, places Holter monitors and downloads telemetry data. • Works in conjunction with other staff to maximize clinic flow. • Intervenes appropriately in emergency situations. • Documents data and changes in the Electronic Medical Record. • Provides assistance and or back up support for administrative tasks including but not limited to scheduling patient visits and performing basic “check-in, check out” activities. • Obtains measurements, patient history and other intake activities. • Stocks and maintains the clinic area including ordering of supplies as needed. • Demonstrates knowledge of pertinent aspects and competency with protocols related to the use of chemical disinfection, high level disinfection, and sterilization if required as part of the responsibilities of the role. • Depending on location, directs High Level Disinfection scope cleaning in accordance with defined HLD processes. • Completes trainings and education including but not limited to Healthcare Provider CPR. • Other job functions as assigned. Knowledge, Skills, and Abilities: • High school diploma required. • Successful completion of a formal Medical Assistant program. • Possesses 0 – 3 years of relevant clinical experience. • Certified Medical Assistant (CMA) (AAMA) or Registered Medical Assistant (RMA) preferred but not required. • Some college, preferred. • Basic Life Support certification from the American Heart Association. • Excellent communication/listening skills. Substantial interpersonal skills sufficient to work effectively with pediatric patients, families, physicians, nursing and other allied health and medical center personnel.
