As directed by the team leader and the Registered Nurse and in partnership with the health care team and patient/ family, performs work processes to accomplish positive patient outcomes, and organizational goals. Provision of care focuses on psychosocial support and activities of daily living. We do 12 hours shifts from 6:30am-7pm and 6:30pm-7am. We are currently hiring for both shifts, Full and Part Time. Responsibilities ESSENTIAL FUNCTIONS AND RESPONSIBILITIES • Actively demonstrates the organization’s values and mission, and conducts oneself at all times in a manner consistent with these values: Safety of our patients and staff, Excellence in all that we do, Respect for everyone all the time, Value in all that we offer and Enthusiasm as our way of life. • Completes 100% of training, in-service, Do-it Groups (DIGs) and Just Do It (JDI) requirements established by MNM and by any professional or licensing organizations.* • Cares for patients demonstrating clinical competence in implementing a plan of care. Maintains basic technician skills, such as obtaining vital signs. Reports observations, including any change in residents condition to Registered Nurse in a timely and accurate manner. • Performs interventions to enhance psychosocial support. Assists residents to Activities. • Provides/assists with care to support physical function for activities of daily living, such as toileting, bathing, feeding, and ambulation which allow residents to achieve their highest practical level of function. • Documents according to facility guidelines to accurately reflect care provided immediately after performing procedure or task.

Registered Nurse (RN)

The Registered Nurse (RN) works under the direction of the Team Leader and is accountable to the Director of Nursing. The RN uses clinical judgment skills to diagnose and treat patient responses to actual or potential health conditions. Assesses monitors, detects, plans and takes appropriate action to prevent potential physiological complications and promotes optimal level of health. Provides medication administration, treatments, and supervision of CENAs performing ADLs. Accountable for integrating care within scope of practice licensure, relevant education and experience. Collaborates with the Care Coordinators, Team Leaders and other team members to ensure communication and timely updates on resident’s change in conditions. Utilizes Policies and Procedures to guide job duties. We do 12 hours shifts from 6:30am-7pm and 6:30pm-7am. We are currently hiring for both shifts, Full and Part Time. Demonstrates effective communication through collegial relationships and interdisciplinary communication. • Demonstrates enhanced time management skills and ability to assist others in order to contribute to overall performance of the team. • Performs other duties consistent with purpose of job as directed by general and unit-specific competency- based objectives
