Assures professional nursing care is provided for all patients in a hospice unit according to established standards and practices. The Team Lead is a team member in a leadership capacity under the direction of Hospice leadership. The Team Lead assists in coordinating unit activities which assure optimal patient care. The Team Lead supports and directs staff members in providing compassionate care and services to patients and families. Independent decision-making is expected in the execution of routine duties. Responsible for coordinating the day-to-day functions for assigned area. Assists with ensuring that departmental goals are achieved, timelines met and service excellence provided in all aspects of work. Subject matter expert. Essential Functions Assigns work; plans and manages priorities, provides technical assistance, oversees staff schedules, monitors quality of work, monitors team projects, mentors staff, provides constructive feedback, oversees staff training, ensures quality improvement, provides leadership feedback on staff performance, and assists with goal setting for the team. Directs patient care activities in the department by making/adjusting staff assignments that ensures optimal patient outcomes. Supervises and assesses clinical practice. Directs and delegates other team members. Coordinates staffing optimal shift scheduling. Assess, plan, implement and evaluate patient care needs. Documents patient care in the healthcare record according to established standards. Directs and delegates care to other team members in accordance with their education, credentials, and demonstrated competence. Ensures own professional effectiveness through education and professional development. Participates in the development of and maintains established hospital policies, procedures and objectives, quality improvement, safety, environmental and infection prevention program Performs other duties as assigned.
