Responsibilities The RN Case Manager - Care Ally works in collaboration with physicians, patients and families to promote quality outcomes for patients with complex medical needs. The Case Manager is responsible for increasing efficient utilization of health care services; identifying chronic or catastrophic cases through the case management process and initiating intensive case management according to program guidelines. The Case Manager is responsible for managing patients in all health care settings to assure the appropriate level-of-care is provided, to prevent inpatient re-admissions and ensure that the patients' medical, environmental and psychosocial needs are met over the continuum of care. The Nurse Case Manager acts as an advocate through the coordination and communication with the health care team and family regarding care planning and implementation of recovery goals. In addition, the case manager will assess and coordinate resources available to patients and maximize the use of health care benefits. Analyze data collected from the predictive modeling tools to identify eligible patients for the Case Management Program. Enroll and manage a case load of patients with complex medical needs, completing case management attestations and initial assessments in accordance with health plan requirements. Develop individualized case management plans with prioritized goals and appropriate interventions in compliance with the risk stratification level requirements. In addition, complete all documentation, member communications, and case closures per policies and procedures. Proficient in knowledge of current Case Management Standards Coordinate Health Care Services with appropriate physicians and health care providers. Manage and provide a comprehensive summary and log of all active Case Management Cases as required by the UM/QA Committee and/or Health Plan. Act as a resource for Utilization Management Staff, physicians, office staff and Patients. Assist with the evaluation and amendment of Case Management Policy and Procedures Attend continuing education sessions and training relating to your job functions as required. Perform additional duties/tasks as assigned. Qualifications LICENSURE/CERTIFICATION: Must be an active Registered Nurse License in the State of Illinois in good standing. Certification in Case Management, preferred. Current CPR certification must be maintained (BLS). EDUCATION: Associate’s degree in nursing required; Bachelor’s degree in nursing preferred. EXPERIENCE: 2-4 years of experience working within the primary care or hospital setting as a Registered Nurse. Previous experience working within case management preferred. The compensation for this role includes a base pay range of $79,040-104,000, with the actual pay determined by factors such as skills, experience, education, certifications, geographic location, and internal equity. QUALIFICATIONS Must possess a current, unencumbered license to practice as a Registered Nurse in the State of Illinois and/or Wisconsin. Audits charts and EHR to ensure daily documentation is done. Assure laboratory and x-ray reports are received and filed for all tests completed. Audits transfers/discharge information to assure accuracy and completeness. Audits charts for deficiencies. IDT-through regularly scheduled chart audits. Assist in developing a written plan of care for each customer that identifies the problems/needs of the customer. Attends and actively participates in resident care plan conference when indicated. Attend Customer and or Family Council meetings when invited. Report all concerns, complaints made by the resident or their family to the Administrator. Recruit, interview, and select the Ladies and Gentlemen of Alden with the approval of the Administrator. Schedule department work hours, personnel, work assignments, etc., to expedite work. Ensure a sufficient number of Ladies and Gentlemen of Alden are available to meet the needs of our customers. Counsel/discipline as requested or as necessary. Complete performance evaluations timely. Make daily rounds to ensure nursing personnel are performing required duties and to ensure that appropriate procedures are being followed. Make physical rounds on all customers daily. Make rounds with physicians as necessary. Submit Workmen’s Compensation forms (once reviewed) to the Business Office Manager within 24 hours after their occurrence. Plan, conduct and participate in In-service Education Programs. Assist in implementing and maintaining an effective orientation program that orients the new Ladies and Gentlemen of Alden to the facility, his/her department and its policies and procedures. Attend and participate in continuing education programs. Place orders for equipment and supplies as necessary following established budgetary guidelines. Maintain budgetary restraints by following established procedures for preparing purchase orders for review by the Administrator prior to placing orders. Maintain a reference library of written nursing materials (i.e., Drug Formulary, State and Federal regulations, etc.) that will assist the nursing service department in meeting the day-to-day needs of the customer. Participate with the Administrator and Admissions Director in selecting appropriate admissions and assignments of rooms. Review nurses’ notes/EHR to ensure they are informative and descriptive of the nursing care being provided, and they reflect the customer’s response to the care. Monitor medication passes and treatment schedules to ensure medications are being administered as ordered and treatments are provided as scheduled.
