Summary The Registered Nurse (RN) 2 is responsible for assessing, planning, implementing and evaluating nursing care for a group of population specific patients for the department /unit, from simple to complex, with multi-system issues in a safe and therapeutic environment. The RN uses knowledge of the principles of growth and development over the life span, assesses data reflective of the patient's status and interprets information needed to identify each patients requirements relative to his/her age specific needs. The RN engages in caring relationships with patient's, families, and significant others, works collaboratively with other health care workers, and uses sound judgments while performing nursing and medical interventions. The RN adheres to specialty standards and organizational standards, policies, procedures, guidelines and protocols. Reports to the Director and/or Nurse Manager. May participate in supervision of Licensed Nursing Assistants and unlicensed personnel. 5 North is a 16 bed respiratory care step down unit. The majority of the patient population consists of those with pneumonia, COPD, acute respiratory failure, sepsis, CHF and DKA patients. The team on 5 north is trained to care for patients on vents, Bipap, Hiflo O2 and chest tubes. As a staff member on 5 North you will work with a complex yet diverse patient population and will be supported by a strong and experienced team of nurses and LNAs. Interviews patients and measures vital signs. Updates patient's medications and allergy lists. Records information in the EMR. Prepares treatment rooms for examination of patients, including stocking and cleaning. Assists provider by preparing patients for physical examinations and by assisting with procedures. Demonstrates clinical skill in practice. Schedules patient appointments and orders referral per practice specific guidelines. Inventories and orders medical supplies and materials for assigned area of responsibility, removes outdated materials and ensures proper labeling. Uses software (Cerner, GroupWise, etc.) following established protocols for documentation, responding to requests, and initiating appropriate tests and orders. Collaborates with physicians to provide cost effective, quality services to the patients requiring anticoagulation therapy. What You’ll Do Provide an anticoagulation management service to EPN/EPS physician network. Obtain PT/INR results from finger stick method or from another source. Complete patient assessment at each visit to include but not be limited to: signs/symptoms of bleeding, signs/symptoms of thrombotic events, significant dietary or medication changes, compliance with the anticoagulation regime, side effects/drug intolerance to anticoagulation therapy, falls, gastrointestinal changes. Accurately adjust anticoagulation dosing within prescribed guidelines using INR reading and clinical assessment as above. Document clearly all dose adjustments and provide documentation to patient at time of visit. Provide educational sessions to outpatient anticoagulation therapy patients and family members to foster adherence, safety and positive clinical outcomes. Utilize written educational materials to provide education. Correctly identify those patients/families requiring additional knowledge, skill or competency to effectively manage the anticoagulation therapy plan. Provide ongoing education and support in an atmosphere that promotes caring and learning. Promptly and accurately identify those situations requiring midlevel or physician supervision, guidance, advice or intervention. Promptly triage anticoagulation patient/family member phone calls. Document all calls/interactions in the anticoagulation navigator in EPIC. Facilitate continuity of care for patients during brief interruptions in anticoagulation therapy (i.e. travel or hospitalization). This may also include providing recommendations for anticoagulation bridging therapy around a procedure when oral anticoagulation must be interrupted. Educational Needs Education: Graduate of an accredited school of nursing. Nurses hired after May 2015, must have a bachelor’s degree in nursing or higher nursing degree or commit to achieving the degree within 5 years. Experience: Cardiology and/or patient medication education preferred. Certification/Licensure: New Hampshire Registered Nurse or Compact State Registered Nurse License. Patients on the Progressive Care Unit need more monitoring and assessment than a medical/surgical unit but are not so unstable that they need to be in ICU. Patients admitted to PCU typically have a diagnosis such as a post-stent placement, post-cardiac cath, heart attack, congestive heart failure (CHF) exacerbations, or pacemaker placement. PCU also provides specialized care for patients with various diagnoses including angina, sub-acute MI, and CHF. Progressive Care nurses complete specialized training required to read and interpret telemetry data to better determine patient care. Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers. Must be comfortable staging pressure injuries according to the NPIAP Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc. Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices Strong preference to having wound certification or be open to getting certified Experience working in a long-term care setting. Overview The DH WRI Medical Assistant Apprenticeship training program is a paid and benefitted full-time intensive 12 weeks of studies in partnership with Colby-Sawyer College. In addition to classroom and virtual instruction, trainees can expect a significant amount of homework and studying outside of the classroom. The course material is taught using a combination of in-person classroom instruction, virtual classes, online activities, skills labs, and shadowing. There is no out-of-pocket cost for those selected to participate in the program. Trainees are provided with all the basic materials to complete the education program, including textbooks, a set of scrubs, and loaned a company laptop. Trainees will need access to reliable internet for virtual classes and out of classroom work. Upon transferring out of the classroom portion, participants will be enrolled in a 2000-hour full-time Registered Apprenticeship through the US Department of Labor.
