RN Hospice 0.8 FTE 64 Hours a Pay Period Days, Hours between 6am-4:30pm Weekends: Every other Holidays: Every other Telecommute: No Position Overview: A registered nurse (RN) is a health care professional skilled in all phases of the nursing process. Delivery of care is directed toward education, promotion, and restoration of health; prevention of disease; and care of the sick and disabled. He/she is adaptable to the needs of the individual and family during health and illness and has the ability to prioritize. The RN assesses, plans, implements and evaluates daily patient care, and is responsible for the direction of staff members assigned to him/her. The RN is responsible and accountable for the delivery of individualized nursing care to patients receiving Hospice interventions. This position may include other duties as assigned. Required to collect, interpret, record and communicate pertinent data that is relevant to the patient’s care through client interview, observation, review of records and consultation with other members of the health care team. Essential Duties & Responsibilities: Conforms to the Nurse Practice Act. Remains up-to-date on Condition of Participation for Hospice, and current Hospice nursing practice, medications, and legal issues related to patient-based services. Follows recommended infection control policies during patient contact. Possesses the ability to assess and interpret all patient findings including all body systems and in-home needs. The RN utilizes data collected to develop an individualized plan of nursing care that reflects the use of current knowledge, available resources, patients’ rights, and communication with the individual, family, and other health care professionals. This plan of care is developed in collaboration with other health care team members and the Hospice Interdisciplinary Team. Collaborates with the physician/provider to develop a medical plan of treatment that meets the health care needs of the patient and family. Revises the plan of treatment with the physician/provider as the patient’s health care needs change. Exercises informed judgment and uses individual competence and qualifications as criteria in seeking consultation, accepting responsibilities, and delegating nursing activities to others. Maintains competence in all technical skills needed to deliver client care (i.e. venipuncture, infusion therapy, aseptic technique, etc.). Reports malfunctioning equipment promptly and removes from the patient care area to maintain a safe environment. Guides and teaches patients/families toward “self-help” independence in recognition and solution of physical, emotional and environmental health problems and accepting responsibility for care. Exhibits ability to make decisions independently, appropriately and considering the well- being of all involved. Acts as a resource person for all members of the health care team. Identifies, labels and dispenses medication accurately and provides patient education Maintains communication with the Hospice office team, RN case Manager and Interdisciplinary Team to assure patient needs are met. Communication includes but is not limited to: scheduling, patient status changes and community needs, etc. Performs nursing activities in an organized and timely manner, maintaining productivity standards. Continually works to gain better understanding of workflow of the department and uses knowledge base to analyze department processes and makes changes to improve efficiencies. Ongoing education and in-depth understanding of Continuous System Improvements (CSI) and supports standard work. Participates in and supports continuous improvement event initiatives. RN is knowledgeable of CMS Condition of Participation for Hospice. Completes documentation and patient assessments according to CMS Rules and Regulations. Supervisory Responsibilities: Supervises home health aide caregivers to assure adequate patient care is provided in accordance with the medical and nursing plan of care. Orients the home health aide to the plan of care for each patient and supervision continues on every two-week basis. Skills and Experience: Required: 2- or 4-year nursing degree. Graduate of an accredited school of nursing. CPR Certification. RN License - MN. RN License - WI. Basic computer skills. Word. Excel. MatrixCare or another database. PowerPoint. The Summer 2025 Nurse Intern is an integral part of the health care team who, under the direction of a professional nurse preceptor, assists in the delivery of patient care in the inpatient departments (Med-Surg-Peds and the Family Birth Center). The Nurse Intern will participate in activities that will strengthen clinical skills, critical thinking, and interdisciplinary communication. The Nurse Intern primarily supports nursing functions as planned and delegated by the professional nurse preceptor who agrees to act as a supervisor and is immediately available to them. Essential Duties & Responsibilities: Completes Winona Health orientation and educational requirements, maintains competency and practices and contributes to Winona Health’s Mission, Vision, and Values. Under the direction of a professional nurse preceptor, they will perform duties related to the care and comfort of patients. The designated tasks are those which have been taught in the academic programs, orientation, and where the individual has demonstrated competence to a designated RN preceptor. Assists with: rooming of patients, bathing, feeding, ambulation, bed repositioning, toileting, personal cares, and vital signs. Obtains and puts away supplies to ensure quality and timely delivery of patient care. Is constantly vigilant to the safety needs of patients, taking necessary precautions as warranted and communicates concerns to the professional nurse in a timely manner. Responsible for inputting all applicable data collection and task completion into the Electronic Medical Record (EMR). All of the Nurse Intern’s documentation must be verified for accuracy and co-signed each shift by the professional RN preceptor, and the RN preceptor must complete and document independent patient assessments as required. Reports any malfunctioning equipment promptly and removes from the patient care area to maintain a safe environment. The Nurse Intern will administer and document administration of medication at the point of care and under the direct supervision of the RN preceptor. Other duties as assigned. The CST is active in the patient care process and collaborates with the RN to deliver quality and safe health care. The CST understands the surgery environment and patient care process. They provide care while meeting physical, psychological, social, spiritual, and educational needs of the patient and family. The CST collaborates with the RN to modify the plan of care based on the needs of the patient and reports changes in the patient’s condition to the RN. Essential Duties & Responsibilities: Keeps updated on current patient care practice, medications, and legal issues. Follows best practice recommendations including but not limited to: AORN, AST, Perioperative Guidelines and organization wide policies and procedures. Works closely with the surgeon while recognizing surgeon preferences and preference card details. In coordination with the RN, assembles the necessary instruments, supplies, and equipment for the surgical procedure. Checks that all equipment is functioning properly before use. Notifies RN of defects, removes defective equipment from the patient care area, labels and documents defect and need for repair in order to maintain a safe environment. Practices safe site surgery techniques including but not limited to time out standard work and stops the line when safety issues or concerns with patient care occur. Performs sponge, needle, and instrument counts with the RN in accordance to policy/procedure. Consults with the RN to revise surgeon preference cards. Demonstrates an understanding of surgical Applies knowledge to set up cases by pre-operative scrubbing, gowning, gloving, instrumentation, sutures, assisting surgeons with gowning and gloving, draping patients, and/or assisting surgeon with draping; intra-operative coordinating and passing of instrumentation and being alert to the needs of the surgeon, post-operative drape removal from patient, removal of contaminated instruments and transport to the decontamination room for processing, safe disposal of all sharps, and proper disposal of trash and linen. Is constantly vigilant to the safety needs of the patient and takes necessary precautions as warranted. Follows recommended infection control/prevention procedures. Follows department specific policies, procedures, standard work and assists in updating the standard work content as needed. Understands the procedure being performed and anticipates the needs of the surgeon. Has the necessary knowledge and ability to ensure quality patient care during the operative procedure and is vigilant for maintenance of the sterile field. Department Specific Requirements: Cleans and prepares instruments for terminal sterilization. Assists other members of team with terminal cleaning of room. Assists in prepping room for the next patient. Participates in the surgical site and patient verification during the time out procedure. Secures tubing, cords, and other sterile accessories. Measures and passes medications, hemostatic agents, and irrigation solutions utilized during the surgical procedure. Holds retractors or instruments as directed by the surgeon. Sponges or suctions the operative site. Applies electrocautery to clamps on bleeders. Cuts suture material as directed by the surgeon. Coordinates the camera or changes out robotic arms/instruments during endoscopic surgery as directed by the surgeon. Prepares and passes off specimen(s) as appropriate. Review the patient’s chart, identifies patient, verifies surgery to be performed with consent forms, and brings the patient to the assigned operating room. Assists with transferring patient to the operating room table. Monitors the comfort of the patient and provides verbal and tactile reassurance to the patient. Assists with positioning the patient, using appropriate equipment and anatomical principles to avoid patient injury. Assists with applying tourniquets and monitors before the procedure. Anticipates additional supplies needed during the procedure. Attends focus board and mandatory staff meetings and completes educational requirements of Winona Health. Other job duties as assigned.
