Description Under the supervision of an Attending Anesthesiologist, this employee administers, monitors and manages patient’s care before and after anesthesia. Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks. Reports to Attending Anesthesiologists. A certain degree of creativity and latitude is required. Nurse Anesthetists will rotate responsibility for daily scheduling of case coverage. Responsibilities In conjunction with attending anesthesiologist, Nurse Anesthetist will perform a thorough and complete pre-anesthesia assessment. Nurse Anesthetist is responsible for obtaining reviewing relevant tests and discussing findings with the Attending. The Nurse Anesthetist shall obtain or verify that an informed consent has been obtained for the planned anesthetic intervention from the patient or legal guardian. Discuss anesthetic options and risks with the patient and/or legal guardian after discussion with Attending Anesthesiologist. Nurse Anesthetists must document in the patient’s medical record that informed consent was obtained. The Nurse Anesthetist, in conjunction with the Anesthesiologist, must formulate a patient-specific plan for anesthesia care. The plan is based upon comprehensive patient assessment, problem analysis, anticipated surgical or therapeutic procedure, patient and surgeon preferences and current anesthesia principles. The Nurse Anesthetist shall induce anesthesia under the supervision of an Attending Anesthesiologist and maintain anesthesia at required levels. The Nurse Anesthetist shall continuously assess the patient’s response to the anesthetic and/or surgical intervention and must immediately notify the Attending Anesthesiologist of significant changes in the patient’s condition and intervene as required, under the supervision of an Attending Anesthesiologist to maintain the patient in a satisfactory physiologic condition. The Nurse Anesthetist must monitor the patient’s physiologic condition as appropriate for the type of anesthesia and specific patient needs: A. Monitor ventilation continuously. Verify intubation of the trachea by auscultation, chest excursion, and confirmation of carbon dioxide in the expired gas. Continuously monitor end-tidal carbon dioxide during controlled or assisted ventilation. Use spirometry and ventilatory pressure monitors. B. Monitor Oxygenation continuously by clinical observation, pulse oximetry, and if indicated, arterial blood gas analysis. C. Monitor cardiovascular status continuously via electrocardiogram and heart sounds. Record blood pressure and heart rate. D. Monitor body temperature continuously on all pediatric patients receiving general anesthesia and when indicated, on all other patients. E. Monitor neuromuscular function and status when neuromuscular blocking agents are administered. F. Monitor and assess the patient positioning and protective measures. The omission of any monitoring standards shall be documented and the reason stated on the patient’s anesthesia record. The Nurse Anesthetist shall be in constant attendance of the patient until another qualified health care provider has accepted the responsibility for care. 6. The Nurse Anesthetist is responsible for maintaining complete, accurate, and timely documentation of pertinent information on the patient’s medical record. Must document all anesthetic interventions, and patient responses. 7. The Nurse Anesthetist is responsible for transferring the responsibility for care of the patient to other qualified providers in a manner that assures continuity of care and patient safety. 8. The Nurse Anesthetist must adhere to appropriate safety precautions, as established within the institution, to minimize the risks of fire, explosion, electrical shock and equipment malfunction. Document on the patient’s medical record that the anesthesia machine and equipment were checked. Prior to use, the Nurse Anesthetist shall inspect the anesthesia machine and monitors according to established guidelines. The Nurse Anesthetist shall check readiness, availability, cleanliness, and working condition of all equipment to be utilized in the administration of the anesthesia care. When the patient is ventilated by an automatic mechanical ventilator, monitor the integrity of the breathing system with a device capable of detecting a disconnection by emitting an audible alarm. Monitor oxygen concentration continuously with an oxygen supply failure alarm system. 9. Precautions shall be taken to minimize the risk of infection to the patient, the Nurse Anesthetist, and other healthcare providers. 10. The Nurse Anesthetist shall participate in the ongoing review and evaluation of the quality and appropriateness of the anesthesia care as a member of the Department of Anesthesiology. Evaluation shall be performed based upon appropriate outcome criteria and reviewed on an ongoing basis. The Nurse Anesthetist shall participate in a continual process of self-evaluation and strive to incorporate new techniques and knowledge into the practice. 11. The Nurse Anesthetist will participate in departmental Grand Rounds, presenting their cases as appropriate. 12. The Nurse Anesthetist shall respect and maintain the basic rights of patients. 13. The Nurse Anesthetist assists in research projects as directed by an anesthesiologist. 14. The Nurse Anesthetist may supervise and teach Student Registered Nurse Anesthetists (SRNAs) Qualifications Nurse Anesthetists must be a graduate of an accredited school of nurse anesthesia and be a Certified Registered Nurse Anesthetist (CRNA) with proof of current certification and with experience in anesthesiology acceptable to the Chairman/Director of Anesthesiology and/or minimum of 1-2 years work experience. Nurse Anesthetist must have completed appropriate accredited training program and clinical rotations and must have graduated from degree program. One year experience in an acute care nursing setting. Must have current valid NYS nursing licenses. Must be certified by the Council of Certification of Nurse Anesthetists. (New graduates are allowed 12 months to comply with the standards of certification); recertification bi-annually according to the standards of the Council of Recertification CNA. Managing regional anesthetic techniques including, but not limited to subarachnoid, epidural, and caudal blocks; plexus, major and peripheral nerve blocks; intravenous regional anesthesia; transtracheal, topical and local infiltration blocks. Performance of any block requires special demonstration of competency and is conferred individually on an annual basis by the Chief of Service at the site/ Performance of any plexus, major and peripheral nerve block also requires approval on any annual basis by the Chief of Regional Anesthesia in addition. Managing the patient’s fluid, blood, electrolyte, and acid-base balance. Evaluation patient response during emergency from anesthesia and instituting pharmacological or supportive treatment to ensure the adequacy of patient recovery from anesthesia and adjuvant drugs. Providing anesthesia care consistent with infection control and anesthetic safety principles in order to prevent the spread of disease and prevent harm to the patient, the anesthetist and others in the anesthetizing environment. Documenting as part of the medical record, all aspects of anesthesia-related care in thorough and timely fashion. Post-Anesthesia Care: Providing a thorough report of the patient’s psychological and physical condition, perianesthetic course and anticipated problems to the post-anesthetic nurse and physician who assume the patient’s care following anesthesia. Providing post-anesthesia follow-up and evaluation of the patient’s response to anesthesia and for potential anesthetic complications, taking appropriate corrective action and requesting consultation whenever indicated. Initiating and administering respiratory support to ensure adequate ventilation and oxygenation in the immediate post anesthesia period. Initiating and administering pharmacological or fluid support of the cardiovascular system during the immediate post anesthesia period as necessary to maintain appropriate physiologic parameters. Perianesthetic And Clinical Support Function: Inserting central intravenous catheters or pulmonary artery catheters requires special demonstration of competency and is conferred individually on an annual basis by the Chief of Service at that site. Inserting Peripheral venous catheters. Inserting arterial catheters and performing arterial puncture to obtain arterial blood samples. Identifying and managing emergency situations, including initiating or participating in cardiopulmonary resuscitation that involves airway maintenance, ventilation, tracheal intubation, pharmacologic cardiovascular support, and management of blood, fluid, and electrolyte and acid-base balance. Providing consultation and implementation of respiratory and ventilator care. Initiating and modifying pain relief therapy utilizing drugs, regional anesthetic techniques or other accepted pain relief modalities as directed by attending anesthesiologist. Selecting or prescribing medication and treatment modalities related to the perianesthetic care of the patient, with consultation or collaboration when appropriate. Accepting additional responsibilities appropriate to the practice setting, which are within the expertise of the individual CRNA. Perform any other procedures, which the CRNA has been educated to perform. POSITION SUMMARY: The CRNA will administer anesthesia for all types of surgical cases, in every setting in which anesthesia is delivered, in accordance with their expertise. CRNA's will administer anesthesia in four general categories: (1) pre-anesthetic preperation and evaluation (2) anesthesia induction, maintainance and emergence, (3) post-anesthesic care and (4) perianesthetic and clinical support functions. POSITION DUTIES: The CRNA will: administer anesthesia for all types of surgical cases, in every setting in which anesthesia is delivered, in accordance with their expertise, state statutes or regulations, and institutional policies: administer anesthesia and anesthesia-related care in four general categories: (1) pre-anesthetic preparation and evaluation (2) anesthesia induction, maintenance and emergence (3) post-anesthesia care and (4) peri-anesthetic and clinical support functions which may include: general, MAC, regional, or local anesthesia. Selecting, obtaining and administering the anesthetic, adjuvant drugs and fluid necessary to manage the anesthetic, to maintain the patient's physiologic homoestasis and to correct abnormal responses to the anesthesia or surgery. Selecting, applying and inserting appropriate non-invasive and invasive monitioring modalities for collecting and interpreting patient physiological data. Managing a patient's airway and pulmonary status using endotracheal intubation, mechnaical ventilation, pharmacological support, respiratory therapy or extubation. Releasing or discharging patients from a post-anesthesia care area and providing post-anesthesia follow-up evaluation and care related to anesthesia side effects or complications. Ordering, initiating or modifying pain relief therapy, through the utilization of drugs, regional anesthetic techniques, or other accepted pain relief modalities, including epidural analgesia. Responding to emergency situations by providing airway management, administration of emergency fluids or drugs, or using basic or advanced cardiac life support techniques. Provide clinical support services outside of the operating room, including but not limited to radiology, endoscopy, emergency department, invasive labs. Upon request or referral for these services, the CRNA will provide consultation and implementation of respiratory and ventilatory care, identify and manage emergency situations, including the initiation or participation in cardiopulmonary resuscitation that involves airway maintenance, ventilation, tracheal intubation, pharmacologic, cardiopulmonary support, management of blood, fluid, electrolyte and acid-base balance. Additional nurse anesthesia responsibilities which are within the expertise of the individual CRNA, as authorized by Medical Staff and stated in the policy and procedure manuals. Non-clinical responsibilities as applicable : Clinical and didactic teaching, supervision and evaluation of students; interdepartmental liaison, member of departmental committees; research participant. Maintains a positive demeanor, proactively helpful to patients, physicians and staff; r emains flexible and alters/accepts various tasks and assignments as they arise, per the Director’s requests or designee. JOB SPECIFICATIONS: Education: Required: Holds a current state license as a registered nurse in the state of Louisiana and complies with any applicable state statutory and/or regulatory requirements concerning the advanced practice of nursing. Graduate from a nurse anesthesia educational program accredited by the Council on Accreditation of Nurse Anesthesia Educational Programs (COA) or its predecessor. License/Certification: Required: Possess certification by the Council on Certification or recertification by the Council on Recertification or their respective predecessors; if pending initial certification, evidence of graduation from an approved nurse anesthesia educational program. Current BLS, PALS and ACLS certifications, as applicable. Skills: Demonstrates knowledge of principles of anesthesia practices. Must be able to operate all anesthesia equipment and to recognize equipment malfunction. Must be able to read written directions and to write legibly. Must be able to communicate effectively with students, residents, physicians, patients, visitors and hospital staff.
