Conducts patient safety rounds utilizing principles of SBAR, follows chain of command to communicate any identified patient or staff safety risks. f. As a Certified Nursing Assistant (CNA) the primary purpose of your job position is to provide your assigned residents with routine daily nursing care in accordance with our established nursing care procedures, and as may be directed by the licensed staff nurse. Participate in planning and following current resident care plan on all residents under his/her care. Complete resident assignments effectively and promptly. Observe changes in resident conditions and promptly report them. Document accurately and completely. Meet personal grooming needs for each resident. Promptly answer call lights and assure that all are placed within reach of the resident. Assure the resident care areas are left clean and in an orderly manner. Methods of identification of residents are maintained. Maintain that all residents are appropriately identified (i.e., I.D. bracelets, etc.). The CNA is responsible for the basic daily care of the client. CNAs will help maintain care records for the client and communicate with other members of the care team. CNAs report to the Administrator and or Alternate Administrator. Essential Functions: 1) To enable patients to stay in their homes by monitoring and recording the client’s physical and mental condition. 2) To maintain a safe, secure and healthy client environment by following necessary standards and procedures. 3) To provide client support and personal services. 4) To protect the home health care company by adhering to professional standards, home care policies and procedures, federal, state, and local requirements. 5) To follow HIPAA guidelines regarding all client PHI.
