Registered Nurse (RN) - Patient Care Navigator - Neurosurgery

Job Description

Navigates patients through their diagnosis and treatment ensuring timely services. Meet with patient and families to provide emotional support, education and guidance to all patients through end stage disease if necessary. Provide medical information, education and review of reports and knowledge of community resources. Responsible for facilitating rapid scheduling and follow-up tracking of patients to ensure completion of their recommended treatment plan. Identify and problem solve any service gaps identified. Promote the Navigator role and prevention, detection and treatment awareness to the community through presentations and participation in related organizations. Coordinates and provides education for all newly diagnosed patients from our campus and outside community. Provides support to clinic nurse roles and maintains many additional job functions as needed.

When doing intake:

Patients needing support from a Functional Navigator have a lifelong movement disorder or epilepsy condition that requires a multidisciplinary team approach to patient care.

Serve as the initial point of contact for new patient referrals as well as referring providers
Screen for appropriateness of referrals
Ensure appropriate and timely scheduling of new patient referrals
Facilitate comprehensive transition of care from referring provider to first appointment
Capture and organize new patient information
Identify and address patient barriers to care
Facilitate scheduling of imaging and lab testing appointments as well as additional referrals
Communicate and educate the oncology care process to new patients
Act as an integral part of the shared decision-making and education process for new oncology patients
Collaborate with team to respond to the needs of outside referring hospitals, clinics, and providers
Effectively leverage and facilitate adoption of electronic resources to elevate the patient experience
Functional Navigator would need to assist on average 57 patients/month: 678 patient/year on a continuum.
There can be up to 4 reps during a functional surgery and the navigator supports device management by facilitating communication between vendors and the health care team to ensure that the patients’ devices are well managed, and the patients’ needs are met.
Patient Care Coordination includes referral intake, neurology, MEG, VEEG, Nuclear Med PET Scan, MRI, Neuro Psych consult, Device Rep Management, Surgery Scheduling, Patient tracking
Patient tracking/care coordination - Coordinates the care of patients who are scheduled to have DBS/VNS device placement. Coordinates all diagnostic appointments and ensures all are completed prior to surgery.
Coordinates device programming dates with neurology - facilitates scheduling appointments for device programming post-surgery.
Gathers data for epilepsy conference and manages dashboard.
Navigator helps coordinate the testing for all epilepsy and movement disorder patients.
