The Discharge Planning Nurse is responsible for daily discharge planning activities in an acute care setting, completing chart reviews for severity of illness and intensity of service criteria. The Lead Discharge Planning Nurses appropriately communicates with the medical staff. This person has direct oversight and is responsible for ensuring the hospital’s compliance with the certification requirements for all third-party payers. They also perform discharge planning duties, complete the verification of insurance benefits and review the requirements needed for inpatient and outpatient services.
Graduate of an accredited school of Registered Nursing, currently licensed to practice nursing in the State of Ohio as a Registered Nurse and BLS are all required. Must have completed a state approved activities course. Continued education in healthcare field, experience with case management and reimbursement related functions are all preferred qualifications.
