Job Description
Bariatric Coordinator oversees and helps organize the clinical activities of the bariatric surgery program. They coordinate communication between all ancillary departments within the bariatric surgery program and Medical staff members, other outpatient and inpatient departments/staff, patient and family. They gather clinical information, results of tests, discharge plans, and changes in patient condition to facilitate patient and staff communication with the bariatric program surgery team.

Hours: Full-time, Monday-Friday, 7:30am-5pm
Requirements: RN, BLS or CPR certification within 90 days of hire, ACLS within 90 days of hire
Pay based on applicable experience; starting at $27.68
Responsibilities
Assessment - Collects pertinent data and information relative to the patient's health or the situation. Recognizes normal and abnormal findings when gathering data., Assists in outpatient clinic by rooming patients, covering the Nurse Desktop and performing other duties typically assigned., Coordinates communications with referring physicians, specialists, patients, families and hospital departments/staff to facilitate timeliness and thoroughness of information shared., Coordination of Care - Plans and evaluates care in collaboration with appropriate disciplines., Diagnosis - Analyzes assessment data to determine actual or potential diagnosis, problems, issues., Documents responses to patient history/screening questions for use by providers during assessments and examinations. Computer documentation proficiency to perform this task in the outpatient setting., Evaluation - Evaluates progress toward attainment of goals and outcomes. Reports data and outcomes to others as appropriate., Implementation - Implements care or work plan in alignment with the plan and approved safety, infection control, and department/organization standards., Knowledge - In collaboration with leaders, actively pursues required knowledge and skills through orientation activities specific to the position, reading current literature and seeking new learning opportunities., Outcomes Identification - Identifies expected outcomes for a plan individualized to the patient or situations., Planning - Develops a plan that prescribes strategies to attain expected, measurable outcomes., Relationships - Establishes effective working relationships with peers, physicians, and other members of the health care / work team. Identifies and confers with appropriate resources regarding patient / work decisions., Relays information between provider(s) and client(s), which may include but is not limited to communications with patients, families, pharmacies, nursing homes, consultancy office staff, insurers, and facilities. At their physician's instruction, communicates provider advice, instructions, orders and results, as applicable, including accurate documentation., Responds to all department-specific emergencies in the clinic to assist with performance of appropriate ACLS treatment., Reviews patient record for complete documentation and compliance with insurance requirements and other regulatory and quality requirements., Supports marketing of program with internal and external customers and primary care providers., Teaching - Employs teaching strategies to promote health and a safe environment., Verify patients' insurance benefits, obtain prior authorizations, schedule and communicate appointments to patients.
Requirements:
