We are currently looking for an experienced MDS Coordinator - LPN/RN to join our team at Westwood Post Acute.

MDS Coordinator - LPN/RN - Full Time - Monday - Friday
- Collects MDS documents within mandated, established and expected time frames.
- Compares data entered against the original MDS document; makes corrections as needed.
- Files the MDS documents in residents' charts.
- Maintains confidentiality of all patient care information.
Requirements:
- Current LPN/RN License
- Experience as a MDS Coordinator in a skilled nursing facility utilizing 3.0
