Description
In an environment of continuous quality improvement, the Licensed Practical Nurse is responsible for performing a variety of patient care activities, supporting the patients' medical and nursing plan of care. Exhibits the MHS Standards of Excellence and exercises strict confidentiality at all times.
Job Functions:
- Assists in all aspects of patient care and non-patient care within their scope of practice.
- Complies with all safety and infection control standards.
- Provides patient care as assigned by charge nurse/team leader.
- Completes required competencies.
- Maintains compliance in RQI.
- Assumes all other duties and responsibilities as necessary.
Medical Oncology/Radiation Additional Essential Functions:
- Performs glucometer blood sugars as patient condition dictates.
- Performs peripheral venipuncture for lab draws following successful training and validation.
- Obtains vital signs, including height, weight, TPR, blood pressure, O2 saturation.
- Obtains and enters current data into the EMR, including meaningful use measures, patient history updates, and clinic summary note each visit.
- Completes medication reconciliation with each clinic visit.
- Performs chart checks in preparation of patient clinic visits.
- Consults social services for patients with elevated psychosocial distress screens.
- Obtains OARRS reporting as requested by ordering providers.
- Participates in community education and health screenings as assigned.
- Administers subcu, IM, and oral medications as prescribed by the provider.
- Provides thorough chemotherapy instruction including resources to patient and obtains their informed consent prior to the scheduled chemotherapy administration.
- Serves as a liaison to the provider to limit unnecessary interruptions to work flow.
- Submits prescriptions following provider authorization and ensures that all prescriptions not electronically submitted are entered into Mosaiq to maintain and up to date medication record.
- Demonstrates general knowledge of prescribed oral chemotherapy agents and associated supportive medications.
- Follows established process for acquisition, inventory, and dispensing sample medications.
- Obtains external diagnostic results including biomolecular testing, pathology, and previous clinical history prior to the provider visit.
- Generates QCL to authorization staff prior to scheduling any patient treatments or diagnostic studies.
- Responds to patient nurseline requests promptly and documents resolution of these requests on the established QCL or in nurse summary note.
- Maintains a clean and orderly environment in the clinic including up to date patient educational material.
Qualifications
Minimum Education/Experience Required:
- Current Licensure in the State of Ohio or if working in WV a WV license as a Licensed Practical Nurse required.
- Must possess a N.A.P.N.E.S., O.O.P.N.E. or State Board of Nursing medication administration card required.
- BLS certification required.
Special Knowledge, Skills, Training:
- Verbal, clerical, numerical abilities, manual dexterity, color vision and near visual acuity.
