Job Description:
As part of an integrated care team leveraging technology to collaborate with and support the needs of the bedside RN and the patient/family virtually. Uses technology to provide remote nursing care to an assigned group of patients in collaboration with the bedside nurse and multidisciplinary team, in accordance with established unit policies, procedures, and protocols. Documents and communicates collaboratively with the bedside nurse regarding the patient condition and all nursing care provided.
This position is located onsite at Facility and is not remote.
Education Qualifications:
Key Responsibilities:
- Assists with completion of admission history, medication history, admission screening tools and additional virtual workflows for assigned patients
- Collaborates with the bedside nurse to initiate, update, evaluate, and revise the plan of care to facilitate the achievement of patient goals and expected outcomes
- Assists with identification of safety concerns and ensures findings are reported to the bedside nurse
- Coordinates and collaborates with the bedside nurse on methods to mitigate safety concerns
- Collaborates with the bedside nurse to complete the patient learning needs assessment and provides patient and family education
- Promotes optimal communication by answering patient/family questions and providing plan of care updates
- In collaboration with bedside nurse, prepares the patient and family for discharge by providing discharge teaching and reviewing discharge instructions and recommended follow-up
- Contributes to the meeting of the mission and goals of the facility and Essentia and meets the requirements of the Joint Commission if applicable and/or other federal, state, and local regulatory or accrediting agencies
- Supports nursing shared governance and the nurse-driven practice model
- Supports system, hospital site and unit-based quality initiatives
- Follows the standard of care in accordance with the level of care requirements per policy
Educational Requirements:
- BSN or ADN degree from an accredited school or college of nursing
Required Qualifications:
- 3 years of Acute Care Nursing Experience
- Proficient with technology
- Epic Experience
- Strong interpersonal skills
- Ability to demonstrate strong critical thinking and problem-solving skills
- Ability to work independently
Preferred Qualifications:
- Previous telephone triage
Licensure/Certification Qualifications:
Certification/Licensure Required:
- Current registered nursing licensure in state(s) where nursing service is provided ND & MN
- Shared Market Service
- Employee will be supporting the West Market facilities
- Will be located within the facility pending hired applicant location in West Market
