8am - 8pm. Coverage area to include Manatee, Sarasota, Charlotte, and DeSoto Counties. Flexible days with weekend requirement.

The Crisis Care LPN provides extraordinary care in the patient’s home. Under physician’s order and RN supervision, the Crisis Care LPN is responsible for one-to-one observation of patient and family needs, outstanding supportive care and emotional support at end of life allowing the patient to remain in their home, enhancing comfort and quality of life. The Crisis Care LPN has the opportunity to use multiple nursing skills in assisting patients at end of life.

Requirements:

Applicants must be a graduate of an approved nursing program and own a current Florida LPN license.
Minimum one year experience in an acute care setting, providing direct patient care; previous experience as a hospice LPN preferred.
