InterCommunity, Inc is a FQHC LA (Federally Qualified Health Center Look Alike). Our health center provides same-day primary care services to every member of the community, regardless of life situation or ability to pay. We also provide a wide array of behavioral health services. Our community Health Care Centers are located at 281 Main St. East Hartford, 16 Coventry St. Hartford and 828 Sullivan Ave., South Windsor. InterCommunity’s Addiction Services offers a similarly wide range of substance use treatment and recovery-supportive services, with many sites in the Hartford area. We provide services across the spectrum of care, including primary care, residential detoxification and substance use treatment, multiple levels of residential care, outpatient substance use disorder and mental health services for adults and children, intensive outpatient programs, employment and community support, mobile crisis evaluations, judicial support services and social rehabilitation.

Summary:
A professional nurse to assume medication responsibilities by monitoring inpatient unit, assessing and medicating patients under the supervision of the Charge Nurse.
Essential Duties & Responsibilities:
- Consistently signs full name and initials in CardEx.
- Completes withdrawal scales accurately and in a timely manner.
- Accurately obtains and transcribes physician orders.
- Accurately documents on control drug sheets, methadone sheets, PRN med record and change of shift narcotic sheet.
- Completes medication administration in a timely manner.
- Accurately assess clients for signs and symptoms of withdrawal and medicates properly.
- Demonstrates safe nursing practice when administering medications.
- Seeks RN advice appropriately.
- Attends discharge team meeting.
- Complies discharge summary within 24 hours. Advises counseling staff of clients current medical and psychiatric condition.
- Demonstrates knowledge of InterCommunity’s policies regarding confidentiality and emergency codes.
- In the event of staff shortage, you may be required to work up past your shift until relief can be found.
*This position is considered an Essential Staff position. Essential Staff are generally defined as employees who are required to report to their designated work area, to ensure the operation of essential functions or programs during an emergency period. Essential Staff are expected to be on-site during assigned shifts. Essential Staff may be required to work additional hours during the event of an emergency or events outside of normal operations to maintain the safety and wellbeing of clients under our care.
Requirements
Education &/Or Experience:
Graduation from an accredited nursing program. Current State of Connecticut LPN licensure required. One (1) year medical experience in a physician’s office required.
Ability to work in an inter-disciplinary treatment setting; Demonstrates the ability to work with substance abuse within the scope of a short-term detoxification program. Ability to work in a cooperative and collaborative manner as a team member with agency associates As well as external teams and with family members and individuals in recovery; ability to prepare clear and concise reports and case histories; considerable interpersonal skills; considerable oral, written, and computer communication skills. Demonstrate flexible and efficient time management; prioritize workload.
