The Registered Nurse works as a nurse home visitor with first time, low-income mothers to implement the Nurse-Family Partnership Program.
JOB DUTIES AND RESPONSIBILITIES:
- Evaluates referrals to determine eligibility for the NFP Program.
- Functions as the designated case manager of 25 clients; is responsible for providing optimum, comprehensive care as outlined in the NFP Program.
- Assesses the physical, emotional, social and environmental needs of mothers and their babies.
- Communicates through counseling and instruction to assist mothers and their babies in attaining targeted goals in maternal and child health, nutrition and parenting.
- Assists women in establishing and attaining goals related to life course development.
- Evaluates progress toward targeted goals and revises the plan of care as appropriate.
- Develops and maintains a working relationship with clients and their families that promotes trust and problem solving competence.
- Demonstrates competency in the assessment and knowledge of growth and development and communication appropriate to the age of the client.
- Consults and collaborates with other professionals/agencies to provide comprehensive support to clients.
- Submits Day Sheets with accurate Billing information in a timely manner.
- Reviews Dates for Evaluation Forms and records due dates of client’s forms for the data collection systems.
- Submits charting and complete and accurate data forms on or before due dates.
- Submit weekly Tracking Sheet with required information.
- Participates in the development and operation of the NFP Program through: attendance at weekly team meetings, attendance at weekly individual conference, compliance with program guidelines and policies, building and maintaining relationships with client referral sources and community partners
- Assumes personal responsibility for ongoing education related to implementation and maintenance of the NFP program.
PHYSICAL AND SENSORY REQUIREMENTS:
Physical Demands: Heavy Work Exertional activity. Occasionally lift and/or carry 100 lbs., frequently lift and/or carry 50 lbs., and stand and/or walk at least 6 hours in an 8-hour work day.
Non-exertional activity: Stooping (bending at the waist) and crouching (stooping and bending at the knees) – required frequently; handling (grasping) – required frequently; and fingering (fine manipulation) – required only occasionally.
Important: The functional capacity to perform heavy work includes the functional capacity to perform medium, light and sedentary work.
Good physical and mental health. Visual and auditory acuity required to provide comprehensive nursing care.
Environmental Conditions:
Inside – Office environment and patient homes.
Outside - Traveling to cases and/or clinics in extreme cold and heat, wet and/or humid conditions, snow, ice and/or fog.
Hazards - Exposure to contagious disease, road and driving, and domestic pets.
EDUCATION:
Graduation from an NLN approved nursing program with a baccalaureate degree and successful passing of the Nursing Board exam are required.
TRAINING AND EXPERIENCE:
A minimum of two (2) years as an RN is required. Maternal-child, Pediatric, and/or Community Health experience is highly recommended.
