Title
Intake Registered Nurse (RN) - Bronson At Home - PRN
The Intake-Referral Nurse is a professional nurse who is qualified to work with a high degree of autonomy as the initial contact point for Bronson at Home. In this role the nurse facilitates the referral process for Bronson at Home. This includes liaison activities between Bronson at Home and physician offices, hospitals and extended care facilities. These activities include intake of referral data, initiating admission processes and coordination of the transition in care settings. Incumbent provides education to hospital staff, extended care facility staff, physicians and other RNs to take verbal physician’s orders and inform families and patients about Bronson at Home services. The Intake-Referral Nurse promotes appropriate use of health care services and shares knowledge of community resources to assist in developing a cost-effective plan of care. Employees providing direct patient care must demonstrate competencies specific to the population served.
Diploma, Associate’s or Bachelor’s degree in Nursing and 3 years’ related experience required
Experience in utilization review, physician’s office, community health, or home care preferred
Licensed Registered Nurse in good standing with the State of Michigan
• Comprehensive knowledge of nursing and health care services
• Comprehensive knowledge of home care, hospice care, the community, and Bronson at Home
• Basic knowledge of reimbursement criteria for skilled home-care, hospice, private duty, and home medical equipment
• Ability to read, write, speak and comprehend English. Second language is desirable but not necessary
• Ability to communicate effectively in a professional manner
• Treats all clients/patients with respect on the phone and face to face
• Treats coworkers with respect on phone and face to face
• Communicates effectively with referral sources, resolves problems and disputes by communicating with internal managers, billing, and clinical staff about complex or potentially difficult referrals
• Communicates to appropriate supervisor problems and complaints, any identified unmet needs as well as positive and complimentary statements about Bronson at Home
• Maintains and respects confidentiality of client, employee and corporate information
Work which produces very high levels of mental/visual fatigue, e.g. CRT work between 70 and 90 percent of the time, and work involving extremely close tolerances and considerable hand/eye coordination for sustained periods of time.
Involves regularly lifting of bulky or moderately heavy weights (i.e., up to 50 pounds), and occasionally assisting with heavier tasks or expending the equivalent effort in pushing, pulling, or otherwise handling material, equipment, and other objects.

• Supports Community Education by maintaining a comprehensive knowledge of community resources and Bronson at Home services and sharing this information with referral sources, providers and consumers..
• Uses knowledge of reimbursement sources and community resources to promote appropriate use of health care services.
• Demonstrates accuracy in data entry and willingness to develop/maintain computer skills.
• Maintains knowledge of case management requirements.
• Within the framework of this job description exhibits understanding of, and compliance with, Conditions of Participation, HIPAA, The Joint Commission standards, intermediary regulations, and other appropriate legal and regulatory codes.
Productivity
• Accurately, timely, and completely documents client referrals, physician orders, and orders for equipment by phone, fax, or one to one basis.
• Confers with referral sources to determine need for private duty, hospice services, or home medical equipment.
Communication:
• Communicates effectively with referral sources, resolves problems and disputes by communicating with internal managers, billing, and clinical staff about complex or potentially difficult referrals.
• Communicates to appropriate supervisor problems and complaints, any identified unmet needs as well as positive and complimentary statements about Bronson at Home.
• Investigates and advises patient and family regarding reimbursement for home care.
• Facilitates safe and confidential transfer of referral information to Bronson at Home business units.
• Assists with coordination of care and ensures flow of information from referral source to clinical staff and/or other business units.
• Maintains and respects confidentiality of client, employee and corporate information.
