. MDS Coordinator reports directly to the facility General Manager
• Maintains MDS schedule within stated MDS time frames and informs all members of Interdisciplinary Team and Education Director of pending MDS assessments
• Creates new assessments on electronic system according to MDS schedule
• Attends weekly, monthly, quarterly and annual MDS meetings as nursing department representative
• Generates goals for interdisciplinary care plan
• Signs MDS for completeness
• Transmits electronic copy of assessments to Centers for Medicare and Medicaid (CMS)
• Creates signs and transmits reentry and discharge tracking forms
• Reconciles all transmission to CMS
• Develops and revises individualized, comprehensive and timely care plans based on appropriate assessments, the MDS assessment and Care Area Assessment analysis
• Is knowledgeable and adept at conducting root cause analysis to develop appropriate care plan interventions
• Creates and transmits any MDS assessments that need correction
• Generates documentation forms for carryover of program goals under the individualized, comprehensive care plan
• Works with C.N.A.

Requirements
Qualifications and Position Requirements: The desired minimum requirements necessary to effectively perform position responsibilities include graduation from an accredited School of Nursing and successful completion of all requirements for Registered Nursing; excellent organizational and time management skills or an acceptable equivalent combination of education and experience, and the ability to communicate in English both orally and in writing, to complete required Nursing Department documentation, and respect for the principles of resident rights, confidentiality, EEO and ADA.
- Must be a Licensed RN
- Minimum of 2 years experience in Skilled Nursing/Healthcare
- Clear understanding of MDS is required
- Experience with MDS Required
