Job Summary and Qualifications
The Perinatal Navigator functions as a support person, advocate and educator for the families whose unborn babies have been diagnosed with congenital birth defects. The navigator serves in his/her role from diagnosis (either the fetal diagnosis, or maternal complication) through the end of active treatment in the postpartum period. The duties of this role include, but are not limited to: · Utilize advanced assessment skills to coordinate care for families of fetuses affected with congenital birth defects and prematurity. · Assess health status and identify healthcare needs for cohort of patients/families. · Collect and analyze data and formulate a comprehensive, family-centered plan of care in collaboration with the medical team and other appropriate healthcare members. · Provide/coordinate healthcare for patients/families in the Maternal Fetal Institute (MFI). · Serve as an educational resource to the patient/family, nursing staff and provider team. · Act as a consultant to multidisciplinary healthcare team members. · Assist patients with appointment scheduling with subspecialty providers · Coordinate and assist patients to remove barriers related to insurance coverage, transportation, child care, finances, language so the patient can focus on the treatment plan. · Schedule patients for private tours of L&D, and /or NICU units. Based on the needs of the patients, tours may include antepartum, postpartum, nursery, CHSU and pediatrics/ · Provide instruction in disease management, medical plan and health/skill teaching to patient and families. · Act as a focal point for evaluating and developing effective patient/family centered evidence-based care strategies for MFI patients. · Attend multidisciplinary meetings as necessary. · Perform direct patient care for patients of the Specialty Obstetrics Referral Clinic (SORC) within the scope of the Registered Nurse Practice Act, with emphasis on nursing process / outcomes for the Obstetric, Woman’s Post-Op and Post- Vaginal/Cesarean Delivery Patient. · Other duties as assigned. |
What qualifications you will need:
- Basic Cardiac Life Support must be obtained within 30 days of employment start date
- Registered Nurse licensed in the state of Texas or compact license
- Bachelors Degree in nursing required, Masters preferred
- Previous Maternal Health experience required; L&D experience preferred
- Fetal Monitoring Certification required
