RN Specialty Care Nurse Navigator – Infectious Disease

Position Summary

The RN Specialty Care Navigator in Infectious Disease coordinates the care of patients receiving ongoing community-based treatment for infectious conditions. Works closely with community hospitals, agencies, and infusion providers to manage the patients’ transition from acute to community-based care settings and coordinate initiation and ongoing management of intravenous anti-infective therapies.

Position Qualifications

Associate of Science in nursing from an accredited institution required. Bachelor of Science or higher degree in Nursing from an accredited institution strongly preferred.
Registered Nurse license in the state of practice (NH, NLC state, MA or both) required. Certification in Ambulatory Care Nursing (RN-BC), Care Coordination and Transition Management or relevant clinical specialty preferred. Nurses are expected to pursue professional certification in area of specialty upon eligibility, or within three (3) years of entering the specialty practice preferred.
Incumbent is required to maintain a valid driver's license throughout duration of employment in the position.
A minimum of six (6) years of profesional nursing experience (RN) in bedside nursing, home health, ambulatory care or case management required. A minimum of one year of acute care experience preferred.
In-depth knowledge of IV management, troubleshooting and complications required.
Working knowledge of pharmacology of common anti-infective therapies required.
Knowledge of non-pharmacological adjunct therapies for pain management preferred.
Case management skills preferred.
Proficiency in management of intravenous therapy and infusions.
Proficiency in telephone triage.
Excellent oral and written communication skills.
Strong organizational, problem-solving, decision-making and critical thinking skills.
Basic computer skills.
Ability to remain tactful and diplomatic while handling stressful and emotional communications.
Ability to provide conflict resolution.
Ability to manage multiple competing demands and prioritize.
Empathetic caregiver, with adaptability for change.
Lifelong learner open to developing new clinical skills as required and requested.
