Job Description:
LPN - CAT Sarasota Desoto County
Licensed in the State of Florida as a Practical Nurse; one year experience working with children and adults that have mental health issues. Valid Florida driver’s license is a requirement.
Special Skills, Knowledge and Abilities:
Knowledge and understanding of the characteristics of children with mental health issues, their families, and family systems. Knowledge of community resources or ability to learn quickly as well as good organizational, time management and communication skills. Ability to work well with community partners and in a team environment is essential. Highly motivated, self-directed, and willing to work a variety of hours. Bilingual preferred.
Essential Functions:
- Conduct nursing assessments for CAT clients.
- Arrange required lab work at specified laboratory and maintain record of results in client medical record.
- Provide clients and families assistance with medication pre-authorization as required and facilitate participation in sample programs and patient assistance programs as needed.
- Manage requests for refills.
- Attend CAT treatment team meetings as a member of the interdisciplinary team.
- Evaluate and document the efficacy, side effects and complications of medications. Report findings to psychiatrist.
- Provide direct medical services and psychoeducation to clients and families on weekly basis in home, community and office.
- Case Management duties in office, home and community.
- Networking with community physicians for further assessment and treatment to ensure continuity of care while establishing contact and rapport with referral source to maximize collaboration.
- Provide coordination with inpatient facilities to ensure medication and medical continuity of care through transition to outpatient status.
- Provide client medication education and health wellness education.
- As necessary, consult with Director of Nursing to maintain medication policies and procedures including transcribing, administering, evaluating, and recording psychotropic medications prescribed by the psychiatrist.
- Maintain medical areas and records according to corporate, State of Florida, and JCAHO standards.
- Review and update medical sections of the medical records as needed.
- Ability to meet productivity standards of at least 50% as set by management
- Perform other duties as assigned.
Work Environment/ Physical Demands:
Contact with a variety of individuals. Outdoor conducting home visits. Sit and stand for extended periods of time. Reach in all directions including above the shoulders. Lift and carry objects weighing up to 30 pounds. Bend, twist, kneel, crouch and stoop. Write legible reports. Read notes, memos, and printed materials. Verbal, auditory, and written capabilities to effectively communicate in an articulate manner.
Position Type, Expected Hours of Work and Travel:
This is a full time, hourly position. Routine days and hours of work are Monday through Friday from 8:30am to 5:00pm.
