FHCSD is one of the top 10 largest federally qualified health centers (FQHCs) in the country. We operate more than 90 sites across San Diego County, including 29 primary care clinics, 23 behavioral health facilities, 10 physical rehabilitation clinics, nine dental clinics, five vision clinics, four outpatient substance use treatment programs, three mobile medical units, two mobile counseling centers, two urgent care centers, and a pharmacy.

Our staff provides care to over 227,000 patients each year, of whom 91% are low-income and 29% are uninsured. FHCSD provides care to all. Services include, but are not limited to adult care, chronic disease management, pediatrics, comprehensive women’s care including obstetrics, dental, vision, case management, physical rehabilitation, speech therapy for children, vaccinations, infectious diseases, behavioral health, substance use counseling and a host of specialty services including cardiology, podiatry, endocrinology, dermatology, among others. FHCSD also offers supportive services to those who are unsheltered and in need of intensive case management.  

The breadth of our clinic locations, services and programs has grown over the last five decades, making us the largest community clinic provider of health care to the uninsured in the county and one of the top 10 largest community clinic organizations in the nation. We are also the largest health care safety-net provider, largest school-based health care provider and the largest mental health provider in the San Diego region.

We are currently seeking a Nurse Practitioner or Physician Assistant to join our team Chula Vista Family Health Center. This location offers a wide range of care to our patients in the surrounding community and has a strong supportive team for our providers.

Clinic Details

Duties and Responsibilities

  • Provide quality and caring medical services to a diverse patient population
  • Perform routine exams, common procedures and screenings
  • Order and interpret laboratory tests, imaging, and point-of-care tests
  • Evaluate patients with complex problems and refer to appropriate specialists
  • Exercise cultural sensitivity and seek assistance with translations if needed
  • Complete all required documentation in accordance with departmental standards
  • Comply with electronic medical record requirements, including medication reconciliation, finalizing notes, clearing out personal pool, and participating in clinic pool
  • Attend all mandatory meetings and minimize incident reports
  • Use resources efficiently and practice medicine in a cost-effective manner
  • Participate in Clinical Outcomes Program
  • Maintain behavioral standards and respectful interactions with colleagues and patients
  • Obtain good peer reviews, attend mandatory meetings and minimize incident reports
  • Work toward meeting organizational targets for all metrics
  • Participate in administrative duties and on-call schedule in a timely manner
  • Must keep all required medical credentials active and in good standing
  • Perform other duties as assigned

Requirements:

  • 2 years of experience in outpatient settings
  • Current unrestricted license to practice in the State of California
  • DEA license
  • Current American Heart Association health care provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training
  • Experience with underserved, low-income, or homeless community members preferred
  • Second language fluency desired, not required

Authorization: 1. Temporary/Relief Primary Care Physician shall complete and return all forms required regarding treatment of patient/youth. Authorization must be documented in the patient/youth medical record in all cases of essential services before considering any non-emergency specialty treatment, or any consultations by specialty Physicians, Physician Assistants, and/or Nurse Practitioners or diagnostic procedures not specifically stated in a prior authorization form. 2. Authorization for Transportation/Transfers: Temporary/Relief Primary Care Physician shall agree that, as required by law, retains full authority to determine the way a patient/youth is transported to the Institutions/Facilities or transferred to other healthcare facilities, after the course of treatment or therapy has been implemented or completed. Temporary/Relief Primary Care Physician shall not transfer a patient/youth to any Institution/Facility without prior written authorization from the appropriate Institution/Facility designee. 3. Experimental and Investigational Drugs and Procedures: Temporary/Relief Primary Care Physician shall not perform on or administer to any patient/youth any experimental or investigational treatment, therapy, procedure or drug. Such treatment, unless it is otherwise permissible under law, is prohibited under Penal Code, Section 3502, and thus, must have prior authorization by the Institution/Facility designee and Office of Legal Affairs. Temporary/Relief Primary Care Physician shall agree to perform or administer only those medical services which are recognized as being in accord with generally accepted professional medical standards, or as being safe and effective for use in the treatment of an illness, injury or condition at issue, subject to the limitations described in this Agreement. 4. Exclusions and Limitations: a. Temporary/Relief Primary Care Physician shall agree that no healthcare service or treatment shall be provided for those conditions specifically listed in California Code of Regulations (CCR), Title 15, Division 3, Chapter 1, Subchapter 4, Article 8, Section 3350.1. Physicians provided by the Vendor may request individual case exceptions for excluded or limited healthcare service or treatment by seeking prior approval of the Institution/Facility's designee. b. Temporary/Relief Primary Care Physician agree that any excluded healthcare service or treatment needed for pre-existing conditions shall be provided only in the event that the condition has become aggravated in such a manner that it poses a significant threat to the patient's/youth's current health and if not treated would result in morbidity and/or mortality. Temporary/Relief Primary Care Physician shall acknowledge that the appeal must be recognized as medically necessary. c. The Medical Standards of Care shall be used as a reference when prior authorization is granted. 5. Clinical Reviews: At the request of the Institution/Facility's designee, Temporary/Relief Primary Care Physician shall perform clinical reviews to determine the quality of clinical care delivered. This includes clinical reviews of index cases (deaths, adverse outcomes etc.) for the purposes of determining clinical quality. As part of the clinical review process, Temporary/Relief Primary Care Physician can also be required to participate in Professional Practice Committee activities and may be called as witnesses in Professional Practice activities. The process of clinical reviews may include the examination of written health records, discussion with Physicians, Physician Assistants, and/or Nurse Practitioners and supervisors and direct patient/youth examinations. The Temporary/Relief Primary Care Physicians who perform clinical reviews are required to be skilled in the preparation of written clinical review documents. 6. Provide a copy of the formulary to be kept on file by the Temporary/Relief Primary Care Physician.
