The role of the RN Case Manager is to ensure that each patient is treated at the appropriate level of care, and that transition to the next level is appropriate and complete. The RN Case Manager will work under the Manager of the department to coordinate a multidisciplinary approach to organize and coordinate effective, efficient, quality care. The CM will work within the multidisciplinary team to develop a transitional plan that is appropriate for the patient as related to benefits, resources, and medical condition. The CM actively uses assessment, planning, implementation, coordination, monitoring, and evaluation. The CM strives for improvements in the quality and cost-efficiency of medical care and the client's quality of life.
Required Knowledge & Skills
Education: Associate Degree or 2 years of college coursework
Experience: Two years of related experience. Requires working knowledge of specialized practices, equipment, and procedures.
Licenses and Certifications
*ALABAMA REGISTERED NURSE LICENSE OR MULTISTATE NURSING LICENSE
*GEORGIA REGISTERED NURSE LICENSE OR MULTISTATE NURSING LICENSE
Qualifications
*Current Georgia and Alabama RN license.
*BSN MSN preferred.
*A minimum of two (2) years clinical experience required, medical surgical experience preferred.
*Case management experience preferred.
*Demonstrated professionalism, integrity, and ethical behavior.
*Demonstrated initiative, critical thinking, and problem solving skills.
*Demonstrated ability to organize, prioritize, and make appropriate decisions following organizational and departmental policies and procedures with minimal supervision.
*Demonstrated interpersonal skills. Excellent verbal and written communication skills.
*Demonstrated ability to analyze and organize information and data.
*Knowledge of utilization review guidelines, case management principles, and regulatory and accreditation standards.
*Ability to function within a synergistic team concept.
