The role of the RN Case Manager is to ensure that each patient is treated at the appropriate level of care, and that transition to the next level is appropriate and complete. The RN Case Manager will work under the Manager of the department to coordinate a multidisciplinary approach to organize and coordinate effective, efficient, quality care. The CM will work within the multidisciplinary team to develop a transitional plan that is appropriate for the patient as related to benefits, resources, and medical condition. The CM actively uses assessment, planning, implementation, coordination, monitoring, and evaluation. The CM strives for improvements in the quality and cost-efficiency of medical care and the client's quality of life. Definitions The Nurse Manager performs nursing duties in accordance with hospital policy/procedure and supervises other nursing personnel who are responsible for patient care. The Nurse Manager assumes 24 hour responsibility for the management of patient care and the personnel who are assigned to that area. This nurse has an in depth knowledge of nursing practice/management and takes responsibility for the development and nurturing of self. This position will include accountability for maintaining high standards of patient care and nursing practice for the entire unit. The Nurse Manager is responsible and accountable for prescribing, delegating and coordinating the nursing care provided on the unit utilizing the nursing process.
