As a RN Care Coordinator, you will be a central figure in patient care, seamlessly navigating the healthcare journey to achieve optimal outcomes and an exceptional patient experience.

Job Requirements

Required

Graduate of an accredited school of Nursing
Minimum two (2) years of acute hospital clinical experience or a Master's degree in Case Management or
Nursing field in lieu of one (1) year experience
Current licensure as a Registered Nurse in the state of Washington (RN-WA)
BLS required within three (3) months of hiring if located within hospital
Ability to pass annual Inter-rater reliability test for Utilization Review product(s) used
Able to apply clinical guidelines to ensure progression of care
Must have critical thinking and problem-solving skills
Collaborate effectively with multiple stakeholders
Professional communication skills
Understand how utilization management and case management programs integrate
Ability to work as a team player and assist other members of the team where needed
Thrive in a fast paced, self-directed environment
Knowledge of CMS standards and requirements
Proficient in prioritizing work and delegating where indicated
Highly organized with excellent time management skills

Preferred

Bachelor's Degree in Nursing (BSN) or related healthcare field
At least five (5) years of nursing experience
Certified Case Manager (CCM), Accredited Case Manager (ACM-RN), or UM Certification
Knowledge of managed care and payer environment

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures.
