NURSE PRACTITIONER MS - PER DIEM Rotating Shift - Cardiac Electrophysiology

About this job:
The Cardiac Electrophysiology (EP) Nurse Practitioner (NP) will practice in three settings: inpatient, outpatient/ambulatory, and procedural settings. The NP will be involved in all aspects of care with the primary goal of ensuring the effective delivery of care to patients.

Inpatient and peri-procedural care
- The Nurse Practitioner will be responsible for obtaining and coordinating the following general items: history and physical, laboratory results, charting of clinical findings, registry documentation, orders, sedation assessment, nursing instructions, consent, and other required elements. The NP, in collaboration with the Laboratory Flow Managers, may be asked to help coordinate admissions and transfers to the hospital. The NP will perform pre-procedure assessments in the recovery room/holding area or outpatient setting as needed.
- The Nurse Practitioner (NP) will be knowledgeable about the Cardiac Electrophysiology Laboratory scheduling, procedure room turnover and availability of resources. They will be called upon to manage patients after their procedures (as appropriate and in collaboration with the consult Fellow) while they await transfer to patient floor or discharge. The NP will assist in family liaison.
- Following procedures, the Nurse Practitioner (NP) will manage vascular access, monitor and manage arrhythmias, pacing, drug therapy, O2 therapy, and other non-cardiac medications, under the supervision of the attending physician. The NP will assist in the evaluation and management of clinical events such as fever, altered hemodynamics, CHF, pulmonary complications, abrupt vessel closure, arrhythmias, access site complications and others. The NP will assist in transfers to and from the intensive care units.
- The Nurse Practitioner (NP) will perform patient education regarding discharge including preventive care and risk factor reduction. They will write discharge orders, prescriptions and complete discharge summaries in collaboration with the attending physician. The NP will act as a liaison between the attending cardiologist and family members and will schedule follow-up care as necessary, including VNA if needed.

Ambulatory care
- The Nurse Practitioner (NP) will collaborate with the team in preparing patients for admissions/procedures. Educate patients regarding a variety of electrophysiology treatments and procedures. Works collaboratively with nursing/clinic staff to assist in the care of patients. This includes, but is not limited to: symptom management, evaluation and management, prescription writing, ordering and interpreting diagnostic studies with review by attending MD, patient calls and questions related to electrophysiology clinical care. The NP will provide care via in person clinic visits, virtual clinic visits, and telehealth.
Procedural care
- The Nurse Practitioner (NP) will be responsible for many aspects of care under the supervision of the attending physician, including but not limited to: cardioversions, suturing, cardiac subcutaneous monitor implant/explant, first assisting during diagnostic EP studies/ablations, device implantation and research procedures performed in the EP laboratory, OR and other clinical areas, as well as fluid and electrolyte management, nutrition, pain management, transfusion therapy, anticoagulation management, education, and post-discharge planning.

General/Working Conditions
- The Nurse Practitioner (NP) participates in departmental and organizational committees and programs as appropriate and attends all required meetings, in-services, and professional trainings. Remains actively involved in continuing education with a commitment to self and departmental growth. Identifies areas for professional growth. Formulates professional goals, objectives and methods for accomplishing these. Supports initiatives within BWH, the Department of Medicine and the Cardiovascular Division.
- Work settings may include ambulatory clinics and treatment rooms, inpatient areas, intensive care units, operating rooms, and/or procedural areas. There may be loud noises, noxious odors, and temperature extremes. A large portion of the workday may be spent either standing or walking and the position may require heavy lifting of equipment and patients.

- The Nurse Practitioner (NP) may be exposed to radiation, potentially infectious bodily fluids, and sharp instruments such as needles and scalpels. Knowledge of Universal Precautions and proper use of surgical sharp instruments may be required. The consequences of contamination from bodily fluids or from being stuck with a sharp instrument include the possibility of physical harm to the employee or others. BWH is a fast paced, dynamic environment with a very large and complex patient volume; exposure to stress in the workplace is expected.


 

Job Summary

The Nurse Practitioner is an advanced practice registered nurse who has acquired specialized knowledge and skills through completion of a nurse practitioner program. The practitioner has the credentials to function in an expanded nursing role and is responsible for the daily management of selected patients. The practitioner has responsibility for the assessment and management of patients, including diagnostic and therapeutic interventions, and the continuous implementation and evaluation of an appropriate plan of care. The Nurse Practitioner works with their collaborating physician in accordance with mutually developed practice guidelines, members of the care team, and is responsible to the Nursing Director or Executive Nursing Director for the area for issues specific to the discipline of nursing.

Satisfactory completion of an approved formal education program that has as its objective the preparation of professional nurses to practice as nurse practitioners.
ANA or other appropriate nursing certification in the area of practice and authorization by the Massachusetts Board of Registration in Nursing to practice in the expanded role. DEA for prescription writing.
Working Conditions - Works in a variety of patient care environments where there may be exposure to communicable disease and hazardous materials.

10. Incorporates the concepts of health maintenance, prevention, and promotion into daily practice through patient education and counseling.
11. Demonstrates awareness of community resources and assists staff, patients, and families to effectively utilize them.

1. Assists in the development, implementation and revision of nurse practitioner guidelines and monitoring requirements for prescription writing in collaboration with their designated physician and Nursing Director or Executive Nursing Director.
2. Evaluates outcomes of own patients’ care and makes necessary revisions in care plans.
3. Seeks and provides consultative services as required.
4. Participates in departmental and organization quality improvement program.
5. Utilizes quality improvement data and research findings to enhance patient care.
-Coordination
1. Demonstrates appropriate collaborative and consultative relationships with other providers within the institution and community.
2. Serves on Nursing and Hospital Committees and assumes responsibility for disseminating and communicating information to staff.
-Budget
1. Participates in setting productivity metrics and in cost-effective management of personnel, time, supplies, and equipment in their area.
2. Collaborates with the Nursing Director/Practice Manager/Executive Nursing Director in the recommendation of appropriate staffing patterns for their clinical areas.
-Personnel Management
1. Participates in selection process of new employees in group or team practice as requested.
2. Provides input into evaluation of other staff as appropriate.
-Education
1. Acts as preceptor for nursing students and staff as requested.
2. Participates in orientation of new staff.
3. Participates in Departmental and organization continuing education programs.
4. Identifies own learning needs and updates clinical knowledge and skills through formal and informal education and review of current literature.
5. Educates lay and/or professional groups through lectures, papers, publications, and educational materials.
-Planning
1. Develops annual objectives and reviews with Nursing Director or Executive Nursing Director and collaborating M.D. at time of performance review and at appropriate intervals. Completes annual credentialing process through the Nursing Department credentialing program.
2. Assists in determining, evaluating, and/or implementing clinical area goals and objectives.
3. Maintains interdisciplinary approach for planning and communicating information for patient care and operation of practice.
4. Maintains current knowledge of legislation that may affect the health care delivery system.

Qualifications

Education & Licensure

  • Master's in Nursing (MSN) (or) Doctor of Nursing Practice (DNP) (or) PhD Degree in Nursing required.  Can this role accept experience in lieu of a degree? No.

  • Completion of a Master’s or post Masters’ degree in Adult program accredited by the Commission on Collegiate Nursing Education (CCNE) or the National League for Nursing Accrediting Commission (NLNAC). 

  • Satisfactory completion of an approved formal education program which has as its objective the preparation of professional nurses to practice as nurse practitioner. 

  • Current Adult/Gerontology (AGNP) or Family Nurse Practitioner (FNP) Certification by an accredited APRN certifying body required.

  • DEA registration required and current APRN License to practice in Massachusetts required.

Experience

  • 1. Minimum 5 years’ nurse practitioner experience caring for a cardiac electrophysiology focused patient population required.

  • 2. Cardiac Implantable Electronic Device (CIED) interrogation and programming experience required.

Nurse Practitioner

Job Summary Reporting to the Director of Hospital Medicine, provides daily evaluation and management to inpatients. This work will be in collaboration with the hospitalist service physician or surgical attending physician, and any physician specialists involved with each patient. The supervising physician of record will be one of the hospitalist physicians for purposes of establishing a collaboration agreement for licensure and prescribing status as well as periodic performance reviews. A second supervising physician may be established for surgical cases. Day-to-day nurse practitioner patient care supervision may be delegated to any one of the on-site hospitalist physicians or surgeons, as applicable. The degree of autonomy of the nurse practitioner’s daily practice will depend on the details of each particular case and the preferences and skills of the NP. The NP will always have access to an on-site hospitalist for help with clinical evaluation, decision-making and logistical back-up. The NP works in accordance with Hospital privileges. Minimum Qualifications Education Completion of a Bachelor’s Degree and an accredited Nurse Practitioner Program. Experience Acute-Care Nursing Practitioner Diploma or Adult Nurse Practitioner graduate with 2 years of acute care or inpatient experience. Licensure and/or Certification Current licensure as nurse practitioner in the Commonwealth of Massachusetts required. National certification with recertification as required every six years. ACLS certification required. BLS Healthcare provider required.
