How you’ll make an impact in this role

Lead the strategic direction and oversee the daily operations for a critical, high-impact case management program.
Drive high-quality patient outcomes by evaluating services, driving the development of personalized care plans, and achieving performance excellence.
Collaborate with physicians and a diverse multidisciplinary team to champion evidence-based care and reduce clinical variation.
Serve as the go-to case management expert, aligning resources and specialized knowledge to support key, system-wide healthcare initiatives.
Resolve complex operational challenges, ensuring a smooth and efficient workflow for the entire case management department.

What minimum requirements you’ll need

Licensure / Certification / Registration:

Registered Nurse obtained prior to hire date or job transfer date required.

Education:

Bachelor's degree in nursing or diploma from an accredited school/college of nursing required.
Master's degree or combination of Bachelor's degree and experience preferred.

Work Experience:

5 years of experience required.
10 years of experience preferred.
2 years of leadership or management experience required.
5 years of leadership or management experience preferred.

What additional preferences we're seeking

Master’s degree
3 - 5 years of Case Management experience in an Acute Care setting
