Responsibilities
- Direct and optimize Case Management and Social Work services, including managing an indirect team of 25.
- Implement new processes and change management initiatives to immediately tackle performance challenges.
- Provide expert clinical oversight and intervention for complex case management, including effective de-escalation strategies.
- Ensure service quality, regulatory compliance, and partnering with teams to reduce clinical variation.
- Serve as a strong, inspirational leader capable of supportive yet tough conversations to develop the team.
Requirements
Licensure / Certification / Registration:
- One or more of the following required:
- Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
- Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
- Case Manager credentialed from the American Case Management Association (ACMA) preferred.
- Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
Education:
- High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required. Master's degree preferred.
Work Experience:
- 5 years of experience required.
- 10 years of experience preferred.
- 2 years of leadership or management experience required.
- 5 years of leadership or management experience preferred.
Additional Preferences
- Registered Nurse obtained prior to hire date or job transfer date
- Master’s degree
- 5+ years of Case Management experience in an Acute Care setting
- 2+ years of leadership/management experience overseeing Case Management teams
