Job Description

Responsible for providing care and services of selected member populations across the continuum of illness. Ensures effective utilization and monitors health care resources. Manages the interdisciplinary team to achieve optimal clinical and resource outcomes. Works with the Supervisor/Manager of Case Management to assess, plan, implement, coordinate, monitor and evaluate services and outcomes to maximize the health of the patient.

Principal Accountabilities

  • Manages, coordinates, and facilitates the care delivered to patients through patient/family assessment and multidisciplinary collaboration.
  • Ensures daily chart review, utilization reviews, coordination of discharge planning activities and comprehensive clinical documentation in the electronic medical record.
  • Provides interdisciplinary patient care rounds, and identifies and works to resolve quality of care issues, barriers to the progression of care and discharge delays.
  • Formulates, coordinates and implements discharge plans and an efficient plan of care to ensure safe, timely and effective discharge plan.
  • Works with the interdisciplinary healthcare team to manage and assist the patient care plan.
  • Performs variance tracking, outcome analysis and identifies problems to assess the effectiveness of the clinical pathways.
  • Servers as a representative of the department and for the hospital community outreach.
  • Performs other related duties as assigned.


Required

QUALIFICATIONS

  • Bachelor's degree in Nursing
  • Valid New Jersey Nursing License Required.


Preferred

  • BSN preferred.
  • Minimum of 3 years of Acute Care experience is strongly preferred.

POSITION SUMMARY The Registered Nurse (RN) delivers direct clinical care across a full spectrum of sexual and reproductive health services, including family planning, abortion care, colposcopy, and LEEP. Practicing within the full scope of New York State RN licensure and in accordance with PPGNY standards of care. ESSENTIAL JOB DUTIES AND RESPONSIBILITIES SRH Clinical Care and ICA Patient Care (90%) • Reviews the patient health history for completeness and records findings. Alerts APC regarding critical information about the patient’s chief complaint, history of present illness, and other current or past medical or social problems. • Provides care to patients receiving family planning, abortion, colposcopy, LEEP, and other GYN services according to PPGNY medical policies and procedures under the direction of APC, or physician. • Provide direct nursing care throughout the abortion care continuum—pre-procedure, intra-procedure, and post-procedure—including preparation, continuous monitoring, and recovery. Assist in the procedure room and recovery area, ensuring clinical stability and emotional support for patients. Administer and monitor medications used to induce sedation in accordance with PPGNY protocols and RN scope of practice. In addition, this position will be eligible for additional benefits consistent with the role. What you will be doing CLINICAL NURSE I - PER DIEM SITE HOURS OF OPERATION: 5:45 AM - 9 PM SHIFT - VARIABLE As an integral member of our Magnet Nursing team, the successful RN candidate will be focused on – patient advocacy and delivering the highest quality of care to our patients ! You will work collaboratively with multidisciplinary health care teams to maintain the standards of professional nursing practices in the clinical setting. You will integrate patient care standards in all clinical activities; participate in performance improvement activities to improve patient safety, quality of care and patient outcomes. The RN will maintain sensitivity and awareness of cultural and religious diversity when addressing patients, visitors, co-workers and other customers. RESPONSIBILITIES Critical care, clinical competence Provide comprehensive pre- and post-operative nursing care to pediatric and adult patients Recognizes and responds appropriately to patient change in condition such as Cardiac rhythm or respiratory distress in the post operative phase. Care for the patient’s post-surgery recovery by observing patients for side effects of anesthesia Monitoring patient’s level of consciousness throughout sedation recovery Communicates appropriate information and patient advocacy regarding pre-operative / post operative conditions to those who require the information including surgeons and anesthesiologist Minimum Requirements BSN from an accredited school of nursing required 1+ years’ of acute care experience NYS RN license and registration required BLS, ACLS, PALS certification required Essential Qualifications HCAHPS/EPIC knowledge preferred Your scope of duties will include critical care clinical competence , incorporating physical and emotional assessments, age specific competency, critical thinking skills, and educating patients and their family members regarding their plan of care and clinical pathway.
