Job Description Summary:

Under supervision and in collaboration with the physician staff, the Advanced Practice Provider (APP) performs physical examinations and treats common episodic and chronic healthcare problems with an emphasis on health maintenance and disease prevention. This position is designed for healthcare professionals, such as nurse practitioners and physician assistants, who possess advanced training and expertise in their respective fields. The primary responsibilities include providing patient care in the clinic setting, performing in-office procedures, and focusing on patient education. As an APP, you will play a critical role in delivering high-quality healthcare services and ensuring excellent patient outcomes.

Job Requirements, Prerequisites and Essential Functions:

Essential Functions and Responsibilities: 

  • Assess patient status by obtaining health history through patient/family interviews and chart reviews; assessing presenting illness, risk factors, and family history, psychosocial situation, and cultural factors; and performing appropriate physical examination.
  • Orders/performs appropriate laboratory diagnostic and other screening tests.  Seeks other information as needed, including consultation with physicians and other clinicians, for evaluation of illness. Integrates data to determine diagnosis and therapeutic plan, including identification of any health risks.
  • Develops and implements treatment plan by prescribing/dispensing medications and/or injections in compliance with medical practice guidelines and state laws.  Instructs patient/family regarding medications and treatments. Educates patients regarding health promotion/illness prevention. Recommends appropriate community resources to meet patient/family needs. Communicates appropriate case management information to other professionals and community agencies.  Prepares documentation for EMR including updating patient records by posting examination and test results, diagnosis, medications, and treatment.
  • Participates in peer reviews, chart reviews, staff education, clinical guideline development, and other continuing education and quality assurance activities to demonstrate compliance with standards, regulations, policies, and procedures.  Acts as consultant/collaborator to clinical team on area of specialty.  Complies with patient confidentiality requirements. Promotes patient advocacy.
  • Collaborates with physicians in managing acute and long-term medical needs of patient. Provides monitoring and continuity of care between visits according to treatment plan including triaging patient calls/e-mails.
  • Perform all other duties as required or assigned.

Assessment:

  • Elicits and documents a complete health history.
  • Performs and documents a complete physical examination.
  • Order appropriate diagnostic tests.
  • Assesses patient’s perception of their health status, personal strengths, support systems, and patient problems that include psycho-social, physical, environmental, economic, and cultural factors.
  • Identifies patients and families at risk for specific healthcare problems.

Diagnosis and Planning:

  • Formulates diagnoses based on health history, physical examination, and diagnostic tests.
  • Formulates and documents health care plans with patients and their families, including treatment, health maintenance education, and counseling.
  • Establishes, reviews, and documents short and long-term goals in collaboration with each patient.
  • Plans comprehensive patient care with other health care providers

Implementation

  • Demonstrates ability to provide appropriate care based on needs of a specific individual, including that individual’s age.
  • Institutes treatment plans based on assessments and interpretations of diagnostic tests and physical findings.
  • Counsels patients according to their individual motivation and compliance.
  • Provides telephone advice and triage of acute and unscheduled patients.
  • Documents all pertinent data and interventions in the patient’s records.
  • Provides refills for routine medications.
  • Institutes appropriate action when reviewing abnormal lab data and diagnostic tests.
  • Consults with other providers, as indicated.

Evaluation

  • Evaluates the patient’s understanding of and compliance with health care plan by regular follow-up and reassessment.
  • Re-evaluates short and long-term goals with patients and participating health professionals.

Education:

  • Patient and Family
    • Identifies the learning needs of the patients and family members.
    • Assesses the patient’s readiness to learn.
    • Provides individualized health education.
    • Develops teaching goals and objectives that meet the patient’s and family health care needs.
  • Staff
    • Functions as a clinical resource person.

Continuity/Coordination:

  • Functions as the health care provider for a specified group of patients.
  • Coordinates patient care activities and health care planning among multiple providers, patients, and their families.
  • Communicates and consults with family, staff and unit personnel prior to and during the patient’s hospitalization. May also participate in discharge planning when appropriate.
  • Promotes ongoing communication with community agencies to facilitate continuity of patient care.

Advocacy:

  • Promotes the participation of patients and families in planning their care.
  • Assists patients in obtaining information necessary to make an informed decision and supports patients in decision-making.
  • Informs patients of the Patient Bill of Rights.
  • Provides referrals to community resources when appropriate.

Qualifications – Nurse Practitioner:

Education:

  • BSN required; MSN preferred
  • Successful completion of advanced practitioner training and certification as a practitioner (general or specialized) from an approved nurse practitioner program

License/Certifications:

  • Current Registered Nurse, as defined by the California Board of Registered Nursing and successful completion from a college or university-based Nurse Practitioner program
  • Active and unrestricted Nurse Practitioner license in state of CA
  • Current Nurse Practitioner Furnishing License and verification of experience to furnish (if applicable)
  • Must hold current national board certification by first day of work and maintain
  • Must hold an active and transferable DEA license to California as required by the clinical specialty
  • Must be eligible for payer enrollment with CMS Medicare and Medicaid
  • BLS Certification (American Heart Association) required

Experience:

  • 2 years of experience in advanced practice or special preferred
  • 2 years in specialty area highly desired
  • Prior experience in a medical office environment preferred

What You’ll Do: Conduct thorough assessments, diagnoses, and treatments for a variety of urgent care conditions. Order and interpret diagnostic tests, including x-rays, labs, and EKGs. Perform typical urgent care procedures, such as laceration repair, I&Ds, and splint applications. Collaborate with a network of peers and specialists to enhance patient outcomes. Triage and manage patients efficiently to maintain a smooth clinic flow with generally 4-5 patients per hour. Qualifications: Physician Assistant: Medical/Master’s degree with a valid state license and national certification. Nurse Practitioner: RN license, NP certification, and a valid state license. Requirements: Valid, unrestricted medical and DEA licenses. Ability to treat patients of all ages. DOT Certification: Must be obtained prior to your start date. Hourly Base Rate: $69.75 - $97.00 Productivity Bonus: Up to an additional 28% Part Time Salary (minimum 2 shifts per week): $72,540 - $100,880 base (at 2 shifts per week) $79,069 - $109,959 with productivity bonus (at 2 shifts per week) Full Time Salary (minimum 3 shifts per week): $108,810 - $151,320 base (at 3 shifts per week) $128,396 - $178,558 with productivity bonus (at 3 shifts per week) Compensation offers include an hourly rate, a wRVU bonus plan, a weekend differential, and a balance of factors such as level of education, experience, work history, and geographic location. What You’ll Do Provide exceptional treatment, collaborative care options (within the scope of urgent care), including physical examinations, assessments, diagnosis and treatment Order and interpret x-rays, lab results and EKGs Triage and manage patients thoroughly and efficiently, maintaining a steady flow to avoid long wait times Perform digital blocks and regional anesthesia; start IV fluids and perform minor surgical procedures, such as wound closure and management, foreign body removal, diagnostic and therapeutic procedures, closed reductions of simple fractures or dislocations of fingers/toes Perform diagnostic and therapeutic procedures, such as aspirations and injection of joints, bursas, and cysts. Start IV fluids Perform closed reductions of simple fractures or dislocations of fingers/toes, including nursemaid’s elbow reductions Apply and modify braces, splints, and other orthopedic appliances Review and manage daily tasks, patient callbacks, and lab results, while on shift About You Medical/Master’s degree and certification PA - Valid state license to practice as a Physician Assistant and current national certification - OR - RN - Valid state license to practice as a Registered Nurse, NP - Valid state license to practice as a Nurse Practitioner and current national certification Current, unrestricted medical license to practice in the state of state Maintain a controlled substance license along with an unrestricted DEA license Ability to consult and treat patients of all ages Experience in ER, urgent care or similar setting (minimum 2 years) Ability to work two full weekends per month (required for full-time clinicians) Maintain confidentiality and impartiality at all times Availability to work in other clinic locations and some holidays as needed (preferred) Perform diagnostic and therapeutic procedures, such as aspirations and injection of joints, bursas, and cysts Perform closed reductions of simple fractures or dislocations of fingers/toes, including nursemaid’s elbow reductions Apply and modify braces, splints, and other orthopedic appliances $72.75-$100.75 / hour Compensation offers include an hourly rate plus a wRVU bonus plan, and a balance of factors such as level of education, experience, work history, and geographic location.
