Primary member of Code Blue, Rapid Response and Code Stroke teams. Performs accurate nursing assessments on all patients to include accurate triage with appropriate level assignment, home medication interrogation, psychiatric history, drug history and domestic safety. Performs targeted clinical assessment including vital signs and discharge assessment in the Electronic Health Record. Mandated reporter. Accurately and safely administers medications utilizing medication administration scanning protocols for the Electronic Health Record. Assesses and documents patient response to medication according to organizational policy. Administers vasoactive intravenous infusions. Models professional behaviors consistent with Service Excellence standards: AIDET, white boards, hourly rounding, patient call backs. Adheres to organizational dress code policy and Code of Conduct. Performs the following procedures: Foley catheter insertion; nasogastric tube insertion; peripheral intravenous line insertion; feeding tube insertion; management of central lines, chest tubes, endotracheal tubes and respiratory ventilators. Performs Phlebotomy as needed to include proper asceptic technique, blood tube order of draw and protocol for blood culture sampling. Recognizes abnormal lab values, reports to MD and implements orders to correct abnormalities. Adheres to and demonstrates personal accountability in implementing and maintaining the most current clinical and service standards. Maintains mandated professional certifications and annual educational requirements. Consistently meets all attendance and tardiness standards. Adheres to all established break and meal period protocols. Plans PTO as indicated and per departmental policy. Overview This position is responsible for the daily work and work-flow within the inpatient units (Medical/Surgical and ICU). The Inpatient Clinical Coordinator (ICC) is a Registered Nurse who works with the Director of Patient Care Services to ensure operational and hospital standards are upheld. The ICC is integrally involved in ensuring that staff, patients, and other department services come together seamlessly to advance patients through their admitted stay and ensure that the patients/caregivers have adequate knowledge of goals of care and discharge plans. Contributes to a nurturing and healthy work environment for all team members, ensuring the highest quality of care is delivered. Responsibilities Daily Operations: Oversees the day-to-day overall function of the inpatient units by ensuring appropriate staffing is in place and assignments are made thoughtfully and strategically, monitors bed management/throughput to ensure timely transitions of care; provides clinical support and staff break relief as needed. Staffing: Ensures appropriate staffing in place each day, works closely with the Director in the development of staffing schedules, being mindful of staff competencies. Is able to step in to bedside care when needed in all areas, and when there are sick calls or emergencies. Works with the staffing office and Director to ensure appropriate staffing for oncoming shifts in compliance with Title 22 regulations and hospital policy. Overview The Certified Nursing Assistant (CNA) is an important member of the patient care team, supporting patients, nurses and also overall unit functioning. The CNA is responsible for helping create and maintain a safe, healing environment for our patients, and a healthy working environment for the care team. The CNA is expected to perform duties autonomously and also at the direction of the RN. The CNA reports to the Director of Patient Services. Responsibilities Assists patients as needed with Activities of Daily Living (ADLs) such as feeding, dressing, and mobilizing. Attends to patient personal hygiene needs such as brushing teeth, bathing, and toileting. Repositions the patient for comfort and per protocol. Also, observes patients for changes in behavior or condition and notifies the nurse of same. Overview SCU (Surgical Care Unit) RN is responsible for the preoperative preparation and recovering of all surgical patients. Responsible to act as an integral team member and keep patients moving through the unit in a safe, efficient, and timely manner. Able to apply critical thinking skills throughput the surgical continuum for each patient under their care. Skilled in post anesthesia recovery care of patient undergoing various types of procedures with different combinations of anesthesia. Responsibilities Opening and closing unit- turn lights on in Waiting Room, unlock front door, remove keys from Pyxis to unlock cabinets, prints daily schedule, gets charts out of cabinet and puts them in order. Checks each patient bay- Suction, airways, ambu bag, O2 tank. Performs QC glucometer. Assigns rooms for patient arrival. Prints H&P 's, orders labs and EKGs where appropriate. Proactively directs the patient plan of care from admission to discharge. Admits and assesses each patient in a timely manner. Follows hospital policies and procedures for patient and procedure verification including all necessary preoperative checks prior to patient going into surgery. Charts allergies and fall risk. Draws labs prn, starts IV prn, notes required documentation on chart and verifies accuracy and timeliness. Gives pre-op meds as ordered. Notes Physician's Orders. Records all medications given on the electronic health record. Provides comfort measures as needed. Safely logs patient belongings and whereabouts. Maintains good communication with patient and family, communicating wait times, delays, etc. Provides education to patients and family both pre-operatively and upon discharge in a manner that its easily understood. Ensures that case management referrals, if any, are completed prior to discharge. Works with Case Management and Social Services as necessary.
