Schedule: Full-Time | No Nights, No Weekends

Position Summary

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our hospice team serving the Montgomery County, Pa area. In this role, the LPN provides high-quality, comfort-focused nursing care to hospice patients in their homes and other care settings, under the direction of a Registered Nurse and in collaboration with the interdisciplinary care team.

Key Responsibilities

Provide skilled nursing care to hospice patients in accordance with the individualized plan of care
Perform nursing tasks within LPN scope of practice under RN supervision
Monitor and document patient status, symptoms, and responses to care
Assist with medication administration and medication reconciliations as ordered
Provide wound care and other treatments per physician and RN direction
Educate patients and families on daily care needs and comfort measures
Communicate changes in patient condition promptly to the RN case manager
Maintain accurate, timely documentation in compliance with hospice regulations
Support patients’ comfort, dignity, and emotional well-being

Qualifications

Current Pennsylvania LPN license in good standing (required)
Hospice, home health, or long-term care experience preferred
Strong clinical assessment and communication skills
Compassionate, patient-focused approach to care
Ability to work independently in the field
Valid driver’s license with reliable transportation
