      Position Summary

      The On-Call Registered Nurse provides after-hours and weekend support to patients and families, including visits, admissions, triage, and coordination of care. The RN responds promptly to calls, assesses needs, offers guidance per hospice standards, and collaborates with the interdisciplinary team to ensure continuity and quality of care.


      Key Responsibilities

      • Perform after-hours and weekend visits and admissions as needed.
      • Respond to incoming calls, triaging needs and directing calls to the DPCS, RN Clinical Supervisor/Manager, or RN Case Manager when appropriate.
      • Refill prescriptions, order supplies and DME, and coordinate services for patients and families.
      • Document all calls, conversations, and outcomes in the EMR system.
      • Receive start-of-shift reports and provide end-of-shift reports according to workflow.
      • Perform initial nursing assessments on new admissions.
      • Complete, coordinate, and revise patient Plans of Care in collaboration with the care team.
      • Maintain accurate, timely, and compliant clinical documentation.
      • Communicate changes in pain/symptom management.
      • Educate families on patient condition, needs, and care expectations.
      • Make referrals to other hospice disciplines as needed and document rationale.
      • Deliver on-call responsibilities and services as assigned.
      • Perform other related duties as needed.

      Requirements


    • Must be a Registered Nurse with hospice experience preferred; telephone triage experience a plus.
    • Active RN license in good standing with the State of Ohio.
    • Strong understanding of hospice philosophy and the unique needs of terminally ill patients.
    • Flexible availability for after-hours, weekend, and on-call schedules.
    • Strong computer skills and ability to navigate EMR and software systems.
    • Ability to handle stressful situations and support grieving families.
    • Must meet all hospice nursing competencies and maintain working knowledge of relevant software.
    • Ability to travel within the Harmony Hospice service area.
